    As always, this book is dedicated to Dianne Rene Litchfield who had the faith to share 32 years of her life with me. She played with me, worked with me, parented with me, wept with me, and opened herself to me. If I have learned anything in this life it is because of her, together with her, or (at least) knowing her hand was in mine as I learned on my own.

    I also acknowledge the nine wonderful children who blessed our home. Although Dianne passed gently on from this world, our home continues to be graced by those wonderful children, and gradually their spouses, and my grand-children. Perhaps I will have the honour of also building relationships with great grand-children. Wow! That would be excellent!

    PREFACE

    Hello there! My name is Bryant Litchfield. I have been a family doctor for about 25 years. My training was paid for by the Canadian Forces. After my internship, I was posted to Canadian Forces Base Borden. My Base Surgeon required me to be the designated physician for:

    1.   the ‘Hole in the Ground’, a military emergency shelter and political communications center in case of National disaster such as nuclear, biological, or chemical warfare or accident; and,

    2.   alcohol and drug rehabilitation for the base.

    But Sir, I argued, I am the only Junior Medical Officer with a spouse and children! If there were a disaster, I would want to be with them. And, I know nothing about drugs or alcohol, because I have never taken a drink in my life! Could I please have the (new, up-and-coming) Sports Medicine responsibilities? Between you and I, it is never a good idea to begin any statement to a Superior Officer with the word BUT. I got a pretty quick answer. Litchfield, I was told, orders are not open to discussion. Then, when I did a preparatory assessment in the ‘Hole in the Ground’ we each had to summarize how our section was prepared in terms of Functional Capacity. I had to present a fairly dismal view of the medical facilities. The medicine in the drugs area included tablets from the Korean War. Some of the penicillin tablets had ‘melted together in a chunk’! The General in charge actually liked my organization and presentation. I thought I had another chance. In a quiet moment with him I told him that I had a wife and five
