Right foot abscess with Diabetes Mellitus Mr.

KGN, 1 years old retired man had a footwear injury on the dorsum of the right foot about a week ago. The wound was oozing pus and blood, surrounding skin was peeled and there was low-grade fever. He was on treatment for Diabetes Mellitus and hypertension. 6 years ago, his left foot had diabetic gangrene with osteomyelitis. Wedge amputation of left middle toe at metatarso-phalangeal joint was done. On examination, his right foot showed diffuse scaling. Plantar skin was hard with cracks and cuts. Peeling of skin on the dorsum reached plantar surface from great toe side with foul smelling purulent discharge. Unhealthy blackish red tissue with two spots of necrosis could be seen below the great toe. Pus culture revealed Staphylococcus infection. Urine showed 15-20 RBC and 1+ albumin. HbA1C was 10.4%. He was started on antibiotics and external Ozone treatment in the form of bagging. Ozone at 60-micrograms/ml concentration was given for 30 minutes everyday for 13 days. Healing was good. Excellent response in diabetic foot with past history of left toe amputation. Ozonation has apparently saved the patient from another digit amputation. Wound almost ready to accept SSG.
