THE SAN BERNARDINO SANTA FE SHOPS ORAL HISTORY PROJECT 1 AM BEING ASKED TO READ THE FOLLOWING MATERIAL TO ENSURE THAT I AM INFORMED OF THE NATURE OF THIS RESEARCH STUDY AND OF HOW I WILL PARTICIPATE IN IT, IF I CONSENT TO DO SO. SIGNING THIS FORM WILL INDICATE THAT I HAVE BEEN SO INFORMED AND THAT I GIVE MY CONSENT. FEDERAL FEGULATIONS REQUIRE WRITTEN INFORMED CONSENT PROIR TO PARTICIATION IN THIS RESEARCH STUDY SO THAT I CAN KNOW THE NATURE AND RISKS OF MY PARTICITPATION AND CAN DECIDE TO PARTICIPATE OR NOT PATICIPATE IN A FREE AND INFORMED MANNER. PURPOSE Lam being invited to participate voluntarily in the San Bernardino Santa Fe Shops oral history project. The purpose of this project is to record, transcribe, and deposit into archives the oral histories of persons whom life stories are closely related to the San Bemardino Santa Fe Shops. These oral histories will be used in future publications, books and other multimedia applications (i.e., websites) of the principle investigators on the history of the Santa Fe Railroad in San Bernardino, California, the history of San ‘Bemardino, the history of railroading in Southern California and the impact, deindustrialization can have on urban centers. SELECTION CRITERIA Lam being invited to participate because either I or a close family member has an intimate connection to the San Bernardino Santa Fe Railroad Depot and the San Bernardino Santa Fe Shops. Approximately 5-20 persons will participate in this study. PROCEDURE(S) If L agree to participate, I will be asked to consent to the following: 1. An oral interview with Kimberly Haysam, Heather Johnson, Christina Perris or Kyle Shell. This interview will be recorded on video or audio tape and transcribed. 2. Lagree to give the researchers copyright to the audio and video taped and transcribed interview. ‘The Interviewer(s) will have the right to use the interview for research, dissertation(s), books, and other publications. 4. Copies of the video recordings and transcripts may be deposited in an archive at the interviewer's discretion 5. The Interviewee will have the opportunity to request a personal copy of the transcript of video recording for his/her own records. 6. The Interviewee retains the right to withdraw from the project, or withdraw his/her transcription from the project at any time. 7. The interviewee will have the opportunity to review the transcribed interview before it is deposited into any archive or used in any publication and will be given the opportunity to request revisions, additions, or deletions at that time RISKS There are no known risks associated with this study. BENEFITS Benefits associated with participation in research will be in preserving a portion of the history to the San Bernardino Santa Fe Shops as recorded through the lives of the participants in this study. CONFIDENTIALITY For the sake of privacy, the Interviewee will choose whether the transcripts and any publication derived from them will use his/ her name or a pseudonym. I prefer that the Interviewer(s) use my name / a pseudonym (Please Circle One). PARTICIPATION COSTS AND SUBJECT COMPENSATION 1. Participants will not be required to incur any expenses related to participation in this project except for approximately two hours of their time during which the interview will take place 2. I MAY ASK QUESTIONS AT ANY TIME AND 1 AM FREE TO WITHDRAW FROM THE PROJECT AT ANY TIME WITHOUT CAUSING BAD FEELINGS. MY PARTICIPATION IN THE PROJECT MAY BE ENDED BY THE INVESTIGATIOR FOR REASONS THAT WOULD BE EXPLAINED. NEW INFORMATION DEVELOPED DURING THE COURSE OF THIS STUDY WHICH MAY AFFECT MY WILLINGNESS TO CONTINUE IN THE RESEARCH PROJECT WILL BE GIVEN TO ME AS IT BECOMES AVAILABLE. THIS CONESNT FORM WILL BE FILED IN AN AREA DESIGNATED BY THE HUMAN SUBJECT COMMITTEE WITH ACCESS RESTRICTED TO THE PRINCIPAL INVESTIGATOR(S), KIMBERLY HAYSAM, HEATHER JOHNSON, CHRISTINA PERRIS AND KYLE SHELL, OR AUTHORIZED REPRESENTATIVE OF THE HISTORY DEPARTMENT.
