Report No.: Site/001 Date: 10 Sept, 2012 Project: Peshraw Tunnel Material Type: Lighting Luminaries & Bulbs Location: Project Site On the 6th Sept. 2012, the board and consultant site staff, inspect the Lighting Luminaries & Bulbs and found the following notes and comments: 1. Contractor to compare and verifying the quantities arrived to site with the drawing, and substitute the missing/addition numbers immediately. 2. The Tunnel Lights is not provided with brackets, contractor to provide (Approved by Manufacturer) Brackets drawing immediately, for approval. 3. Materials that inspected on site: a. 600 Pcs of 250W (THORN Tunnel Lighting) Module: 600 Lights are (7824E 250W HST M32, Made in Sweden). And (7824E 250W HST 2M32, Made in Sweden). b. 100 Pcs of 400W (THORN Tunnel Lighting) Module: 7824E 400W HST 2M32, Made in Sweden. c. 80 Pcs of 400W (ORACLE Street Light). Module: ORACLE 2C 400W HID 230V CL2 EFL, Made in EU CE. d. 600 Pcs Bulbs 250W (OSRAM), Made in Brazil. Module: VIALOX NAV T 4Y 250W. e. 180 Pcs Bulbs 400W (OSRAM), Made in Slovakia. Module: VIALOX NAV T 4Y 400W.
