    Students who wish to petition for a makeup exam must fill out this form and hand in to OSA within three working days of missing the exam. Please attach any necessary documents to verify the authentication of the reason for absence. Students are notified of the make-up exam date after the decision regarding the case has been made. You are expected to be prepared to take the exam on the specified date even if the notice is sent a day in advance. NOTICE WILL BE SENT ONLY VIA EMAIL. Students who miss a scheduled make-up exam will not be given a second chance.
