R Rated Movie Consent Form

In the upcoming weeks, the students will be learning about the major religions
around the world: Christianity, Islam, Judaism, Buddhism, and Hinduism. This is a
small sampling of the multitude of religions with similar teachings of peace and
love. However, religious teachings are sometimes twisted resulting violence in the
name of God.

The following movie, Religulous, is rated R due a brief nude scene. However, the
students will NOT be exposed to it. Instead, well watch the movie for examples of
outlandish and twisted religious beliefs that could lead to violent religious conflicts
in today and in the future.

If you are NOT comfortable with your child seeing the movie then please send this
signed form back for verification. Thank you.
