Gentlemen,
We would like to submit our offer for the supply of labor, tools, materials, equipment and supervision
for the above subject as follows;

1.0 SCOPE OF WORK :
1.1 Mobilization/Demobilization
1.2 Coordination with the client.
1.3 Conduct testing and inspection
Insulation Resistance Test
Polarization Index Test
Winding Resistance Test
Polarization Index Test
Insulation Power Factor Test
Oil Dielectric Breakdown Voltage Test
1.4 Perform degassing of transfomer
1.5 Submit test results and evaluation
1.6 Demobilization


2.0 TERMS & CONDITION:
2.1 Power Supply shall be provided by GTSI/client.
2.2 Additional Oil shall be provided by GTSI/client.
2.3 Parts found defective during course of service shall be provided by GTSI/client.
2.4 Additional scope of work not covered in our offer shall be charged additional to GTSI/client.
2.5 Our work shall start upon received of P.O or N.T.P.


3.0 PRICE :
3.1 Mobilization/demobilization ----- Php 15,000.00
3.2 Degassing of Transformer ----- Php 25,000.00
3.3 Testing (Initial/Final) ----- Php 30,000.00
----- Php 70,000.00
12% VAT ----- Php 8,400.00
TOTAL ----- Php 78,400.00
