Treatment Method: Provide a description of methodology.

The goal of this program is to move each client through the stages of change, ie:

use the information and skills independently.
Interventions Used: Provide a description of Interventions.
The Transtheoretical Model is an integrative, biopsychosocial model which brings key

model to conceptualize the process of intentional behavioral change. Skill-building and
experiential activities reinforce the concepts.

Rationale for using this EBPP. Rationale for using a modification of the EBPP.
"This program is designed to engage, motivate, and stimulate processes of change in
clients at all stages of recovery." Since we are a small, rural program and have small
numbers but often treat a wide-range of specific substances as well as issues, this program
is perfect for us.

Describe the process for determining and documenting discharge from EBPP?
At each group meeting, the Counselor evaluates the client's progress toward the goals of
the program that are outlined in his/her treatment plan. The Counselor also evaluates client
progress toward discharge in weekly staffing meetings and in clinical supervision meetings.
These periodic assessments are documented in the client chart. When the goals are
sufficiently met, discharge is planned and carried out.
