In consideration of being allowed to participate in any way in the Albuquerque Fusion eXploration 205 (or the ABQFX 2015 or
ABQFX! progra"# related e$ents and acti$ities# the %ndersigned ac&nowledges# appreciates# and agrees that'
1. I %nderstand that the Alb%(%er(%e F%sion )*ploration is organi+ed by ,%+anne ,l%i+er and Anna -ongs# two indi$id%als.
2. /he ris& of in0%ry fro" the acti$ities in$ol$ed in these progra"s is significant# incl%ding the potential for per"anent disability
and death# and while partic%lar r%les# e(%ip"ent# and personal discipline "ay red%ce this ris&# the ris& of serio%s in0%ry to
"e does e*ist1 and#
3. I &nowingly and freely ass%"e all s%ch ris&s# both &nown and %n&nown# e$en if arising fro" the negligence of the 2eleasees

7. I %nderstand that photography# incl%ding flash photography# and other recordings# will ta&e place at the ABQFX 2015. I
hereby a%thori+e ABQFX 2015 to photograph# $ideotape# or %se any other electronic "ethod of recording "y li&eness
and3or $oice to be %sed at the ABQFX9s discretion in ABQFX:related p%blications and3or web sites. /he photographs and3or
$ideo footage will not be digitally "anip%lated to change its content. I gi$e ABQFX the absol%te right and per"ission# to
"a&e# copyright# and3or %se# re:%se# or p%blish said photographs3$ideo footage of "e in which I "ay be incl%ded in whole
or in part# and wai$e any right to inspect and3or appro$e the finished printed "aterials# $ideos and3or web sites where "y
i"age appears. I f%rther consent that "y na"e and identity "ay be re$ealed therein or by descripti$e te*t or co""entary. I
%nderstand that all s%ch recordings# in whate$er "edi%"# shall re"ain the property of ABQFX. I wai$e any right to
co"pensation for "y appearance in these printed doc%"ents# $ideos or websites in any and all f%t%re %ses of the
photographs and3or $ideo footage.
