I, Nur Fatihah binti Mohd Ghani the OCCUPATIONAL SAFETY AND HEALTH ACT,
1994 SECTION 29 Safety and Health Officer appointee of G Drink Cooperation hereby
appoint you Shary ervina binti Jasni as the Section 29 (Safety and Health Officer)
appointee responsible for the area known as G Drink Cooperation.
In terms of this appointment, you are responsible for Occupational Safety and Health
matters at the aforementioned area. In order to ensure that you meet this responsibility
you must familiarise yourself with the Occupational Safety and Health Act and its
Regulations. You are also required to ensure that all statutory requirements are met at all
times.
You may not further assign this duty.
Your appointment is valid from 15th September 2012 to 15th October 2013.
You shall report (Time Period) directly to myself on all occupational health and safety
matters arising out of G Drink Cooperation.
