High School Algebra I
Purpose:
The purpose of this formative assessment is to give the teacher a better understanding of how
well the students in the class are remembering and understanding the unit being taught. This
assessment will provide the teacher with some insight into the students understanding of
functions, domain and range, and reading/interpreting graphs. This assessment will consist of
three sections: a multiple choice section, a matching section, and a fill-in-the-blank section.
Upon completion of the assessment, the teacher will grade the assessment and note any areas
where the students seemed to struggle. The teacher will hand back the assessment and go over it
with the students in class to help them understand what they did wrong and identify and areas
where more time needs to be spent on instruction.

Directions:
Over the past week we have been discussing functions in class. We have worked on
identifying functions from non-functions, looked at proper function notation, found the domain
and range of functions, and used graphs to interpret values of functions at points as well as
determine the domain and range from the graph. Now it is your turn to prove to me that you
understand all of these concepts we have been learning in class. I will use the results from the
class to see if we are ready to move on to our next unit or if we need to spend more time on this
unit. After I grade this test I will hand it back and go over the correct answers in class. I will
answer any questions that arise in our discussion of the correct answers.
This assessment is a short one: only 20 questions! The test consists of 5 Fill-in-the-blank
questions, 7 True/False questions, and 8 Multiple Choice questions. Each question is worth 1
point for a total of 20 points.
Immediately after each question you will find a spot to mark if you were sure or unsure about
your answer to the question. If you were confident about the answer you chose, put an X on
the line next to sure. If you werent so confident with your answer to the question, put an X
on the line next to unsure. The Xs you put on the sure or unsure line will not be grades;
they are only for you and I to understand how confident you are with the material.
You will have the rest of the class period to finish this assessment. If you do not complete it
in this time period we can find time to finish it. Dont forget to put your name and the date in the
upper right-hand corner of the page. Take your time, read each question carefully, and consider
all the options when reading the questions. If you have any questions, feel free to come ask me
and I will do my best to help. When you are done, please bring the assessment to my desk. If
there is time left, you can sit quietly at your desk and do homework for another class.
