Abstract
BACKGROUND: Respiratorytractinfectionsarecommonandremainamajorsourceofmorbidity,mortalityand
economiccostworldwide,despiteadvancesinmodernmedicine.Onetreatmentapproachistononspecifically
increasetheimmuneresponseoraugmentinnatedefensemechanismsthroughtheuseofbacteriallysates.
PolyvalentMechanicalBacterialLysate(PMBL)isabacteriallysatemadefromawiderangeofpathogenicbacteria,
includingallofthemostcommonlyoccurringpathogensoftheupperandlowerrespiratorytractobtainedby
mechanicallysis.
AIM: TotesttheavailableevidencethatPMBLisabletopreventrespiratorytractinfections.
METHODS: AnumberofstudiesinvestigatingrandomizedcomparisonsofPMBL(active)withplaceboorno
treatment(control)wereselectedforanalysis.Theprimaryoutcomemeasurewasthepreventionofexacerbationsor
acuterespiratorytractinfection.Theresultswereexpressedasrelativerisk(RR)andthenumberofpatientsneeded
totreatforonetobenefit(NNTB).
RESULTS: Datafrom2557patientsfrom15randomizedclinicaltrials(RCTs)wasinvestigated.PMBLinduceda
significantreductionofinfectionsvsplacebo(RR0.51395%CI0.7220.303p=0.00).TheNNTBwas1.15.The
RRwasalwaysinfavorofPMBL(inrecurrentrespiratoryinfectionsotherthanCOPD,chronicbronchitisand
tuberculosis,RR0.50295%CI0.8240.181inchildrenRR2.20495%CI3.2601.147inCOPDorchronic
bronchitis,RR0.40495%CI0.8640.057intuberculosis,RR0.50295%CI0.8900.114).
CONCLUSIONS: TheresultsofthepresentmetaanalysissuggestthatPBMLiseffectiveinbothinchildrenandin
adultsinpreventingrespiratorytractinfections.OurcurrentmetaanalysisshowsthatthereisatrendwithPBML
towardclinicallysignificantresultsinpatientswithCOPDbutitdidnotquiteachievestatisticalsignificanceduetothe
smallnumberofCOPDstudies.
