Sign and date your forms after you
review them to make sure they are correct. Arrange your court case forms and your Fee Waiver forms into three complete sets in the order that they were listed on the printing page.
File your forms. Take all three sets
of your forms to the Clerks office in the Court printed on your forms. Keep one set of forms for your records.
What happens next?
When you file your forms, either a decision will be made on your application at that time, or you will be notified by mail in the next two weeks. The decision will either be that your Fee Waiver has been accepted, has been denied, or that there will be a court hearing on the matter. Note: If you are given a Fee Waiver based on the information you entered on your forms, and the information was not true, you may face certain penalties and may have to pay the full amount of the filing fee. Also, if you stated on your Fee Waiver that you have no income, you may be required to file a declaration under penalty of perjury. Filing a declaration means you are swearing under oath that you are being truthful about your financial situation.
