1. My name in my Birth Certificate is Helen Marie Sarte y Velez. A
certified true copy of my Birth Certificate is attached as Annex A.
2. I am listed in the Roll of Attorneys (No. 54345, of San Antonio City)
erroneously as Helene Maria Beatrice V. Santi. A certification from
the Supreme Court of this fact is attached as Annex B.
3. The names Helen Marie Sarte y Velez and Helen Marie Beatrice V.
Santi refer to one person: I am that person.
4. I am executing this Affidavit for whatever legal purpose it may
serve.
