Biosocialphysical
The Client
The client is Allen Riddell (pseudonym), a 26 year Caucasian old male who lives with his
mother and three of his younger siblings, and works as a manager at Papa Romano's Pizzeria. Allen has
never been married, and has not had any children with his longtime girlfriend. Mr. Riddell is coming to
see a social worker because of his drug addiction. The client has admitted to speaking to numerous
social workers in the past for the same reason.
Family of Origin History
Allen's biological parents were both twenty years old when he was born. The pregnancy was
unexpected, but his parents were excited about the news. After Allen was born, his parents decided it
was best to get married. He has four siblings: Adam, 23 years old; Marisa, 19 years old; Alex, 17 years
old; and Nick, 9 years old. Other than his youngest sibling, Nick, they all share the same set of parents.
Allen's parents eventually got divorced when he was 13 years old because of the affairs both of his
parents were having during the marriage. His youngest brother is his half-brother from his father's
remarriage. Allen's mother is currently 46 years old, and she went to Oakland University to obtain her
education degree. However, she has barely used her degree, and she currently works as a receptionist at
a local church. Allen's father did not attend college. His father was fortunate enough to get a career in
the automobile industry, and he now works at Roche making a considerable amount of money. Allen's
eldest sibling went to college at Columbia University in Chicago, and he recently graduated with his
Bachelor's degree in film making this past year. His sister, Marisa currently attends Grand Valley State
University where she is still studying the required general prerequisites to get into the science program.
His brother, Alex, spends half of his day at the public high school and the other half at a specialized
school for extraordinary students who test high enough on a specific test. Allen's parents both appear to
be in good health, but Allen spoke about alcoholism and cancer affecting, and even killing, some of the
members of his extended family. He does not know of any sexual abuse in the family.

According to Allen, the atmosphere in the family home growing up was filled with tension.
Once his parents finally ended their marriage, he began to act out. Allen fought with both of his parents
often, and he was kicked out of his mother and father's home a number of times. The client's father has
used extensive force on Allen during various altercations between the two. One time Allen felt it was
necessary to pull a kitchen knife out and point it at his father. Allen recalls telling his father to never
touch him ever again while holding the knife. He remembers feeling empowered by the situation, but
he did get kicked out of the family home for this action. He eventually ended up living at a friend's
house for a few years. During those years, Allen, lost contact with both of his parents. The client said
that his friend's parent have become his family and support system. They were the ones to help him
enroll in college, and they even bought him a laptop for school work. Allen said that he felt loved and
accepted by them, and that this love was truly unconditional. It was a good feeling for Allen. Although,
they were very passive in their parenting style by allowing Allen, and her other children, to abuse
substances openly and throw parties. He lived at his friend's house for almost two years before
reaching out to his mother. It was not until the first time Allen sought treatment for his drug usage that
he spoke with his mother after that long period of time. This moment changed the relationship between
them, and they have stayed in contact ever since. The client currently resides at his mother's house
while he is saving money to move out. Allen works a lot of hours, including most holidays, so the only
ones that he always attends is Christmas Eve, Christmas, and Easter.
Every Christmas Eve he spends the afternoon with his girlfriend's family at the senior nursing
home where her great grandmother resides. Afterwards, they both head over to his aunt Ancy's house
where his entire extended family from his mother's side gets together. It is a tradition that they eat king
crab legs for dinner. They also exchange gifts in a funny creative manner to make the process
enjoyable. On Christmas day the client and his girlfriend bounce from house to house. Since both he
and his girlfriend are from divorced families, Christmas day is usually dedicated to spending time with

his father's side and her father's side. Every Easter the couple spends time at his grandparent's home
with everyone else on his mother's side of the family. The night always end with a few hours of card
playing. The client claims that he usually wins some money in poker or other card games.
Luckily, there hashave not been too many deaths of family members that Allen has had to deal
with thus far. However, the most recent death was his cousin, Steven, who died in war in Afghanistan a
few years ago. Unfortunately, this event occurred on Christmas Eve when everyone was at dinner. The
phone rang and the horrible news arrived shortly afterwards. That Christmas Eve was much different
than the previous holidays. Most of the family drank more wine and beer than usual. Based on the
remarks Allen has stated, it appears that his family does not deal well with deaths, and they often look
to alcohol to resolve their issues. Allen admits that his family is strong Italians that love to drink wine a
lot. What are their beliefs about death?
Developmental History
To Allens knowledge, the client was carried to term, and the pregnancy was smooth. He was
birthed vaginally, and there were no presenting issues. He was breast fed by his mother while he was a
baby. The client was unable to tell me about any major milestones that occurred during his
developmental years, but he did say that his mother told him he developed at a normal rate.
Health History
Allen's first and most extreme medical emergency was when his kidney's failed at the age of
eight. Allen spent three months in the hospital. He has had no surgeries to date. Allen was diagnosed
and prescribed medication for clinical depression and attention deficit disorder. He no longer takes
medication for either disorder. Allen has been involved in seven car accidents. One of those accidents
leadled to a broken wrist and multiple broken fingers when he was driving a few friends to another
friend's house. Allen fractured his left leg during a skateboarding accident in his childhood. Allen was
diagnosed with a substance abuse disorder during adulthood, and he currently is prescribed Suboxone

to relieve the symptoms of drug addiction. Allen is not allergic to any medication to his knowledge.
Relationship/Sexual History
Allen had a group of four or five friends that hung out and kept in touch for nearly fifteen years
or so. Allen talks to his childhood friends on occasion through social media websites, such as
Facebook. Allen has been in a committed relationship with his girlfriend for eight years. They met
when Allen was eighteen years old, and she was fifteen years old. Allen was twelve years old when he
had his first kiss and eighteen when had intercourse for the first time. He lost his virginity to his current
girlfriend, and vice versa. Although, he has had intercourse with one other person right after having
intercourse for the first time. Allen enjoys having sex and has no problems with having an orgasm.
Allen's girlfriend has been getting the birth control injections for the last six years. They have not had
any pregnancy scares. Allen is annually checked for sexually transmitted diseases, and he said that he
has not had any positive results thus far.
Allen reports having serious relationship issues lately. He stated that his relationship lacks
affection, intimacy, and passion. The client mentioned that his relationship was great for about two
years and then they both became heavily addicted to drugs. They still stayed by each other's side and
bailed each other out of bad situations or took the fall for one another. He called their relationship then
as ride or die. The past three and half years off of heroin were shaky and unpredictable. He assured
me that he still loved his girlfriend and wants to marry her and have children with her, but they need to
get counseling and change things first. Allen complained that he does not have sex enough and that
when he does have sex it is not good enough. He does not think his girlfriend is attracted to him
anymore and that is affecting their relationship. He admitted that he has been frustrated recently and
has said a bunch of things he did not mean to his girlfriend. Also, he complained that she not get ready
or try to impress him anymore and just gave up. Allen fears that the person he has been with for the last
eight years and all of the plans they had for their future together may not actually happen. He is scared

of not being with her.
Family of Creation History
Allen is in a committed relationship with his current girlfriend for eight years. They are not yet
married. She is attending college and on the track to obtain her master's degree in social work. They
plan on getting married once she finished school and they are more financially stable. Allen hopes to
have two children, one girl and one boy, one day in the far future. He stated that he loved his girlfriend
very much, and that he knew she was the one for him. He admits to having minor physical fights with
his significant other while they were using drugs. He assured me that he has not placed his hands on her
in a very long time. The client hopes to repair some of the areas the couple has broken during the last
few years. He also hopes to make their relationship stronger than ever after seeking professional
assistance. He would like to discuss these issues during our sessions, and if needed, he has thought
about looking into a couples counselor.
Substance Use/Abuse History
Allen described many of his family members as battling alcoholism. Allen is included in that as
well. Allen and his girlfriend have both struggled with alcoholism and drug addiction. They both
started experimenting with prescription pills years ago. Allen was eighteen years old when he started
abusing prescription pills, alcohol, and marijuana. They tried many different kinds of prescription pills.
The client stated that this time period was possibly the happiest time of his life. In a matter of months,
they were addicted to a pain killer called Vicodin. For the next two and half years, Allen was held
captive by this pill. Allen and his girlfriend were introduced to heroin by accident. The client was
twenty-one years old at the time. They were told the powder was something else when as a matter of
fact it was heroin. Allen spent the next two and half years addicted to heroin. They lost most of their
friends and family, and they relied on each other to survive.
Allen went into an inpatient rehabilitation program in 2009, while his girlfriend attempted to

detox at home. Allen was only there for nine days before his girlfriend was able to convince him to
come home and get high with her. Allen's girlfriend's family performed an intervention for the two of
them in late 2010. Allen's girlfriend went to an inpatient rehabilitation center that day, but Allen was
not able to get in for a few days. Allen was not able to detox at home and after 24 hours of detoxing he
stole money and the family car and went to get high. The car was reported stolen and the client ran
from the cops for a couple of hours before giving up. Allen went to his grandparents' house to end the
chase, and he was arrested in front of his entire family. Allen was in jail for three weeks, and he went to
an inpatient rehabilitation center for forty days after that. Allen and his girlfriend are still currently
together, and other than a few moments of weakness, they have been able to stay clean. Allen stated
that he has been struggling a lot more lately. The client states that he has used drugs about ten times the
past three months. He admits to using alcohol and prescription pills. Allen is concerned about the
potential path he is going to go down if he does not get help now. Allen is sick of using drugs, but he
said that he is not sure how he will be able to live life without using drugs and alcohol. He fears he will
never be able to stay completely clean and sober. Allen is addicted to nicotine, and he smokes about a
pack of cigarettes daily. While Allen was clean he admits to being addicted to gambling. Allen said that
he went to the casino or poker room everyday. He would go more than once on occasion. Also, Allen
struggled with binge eating while he was off drugs, which he believes he has control over now.
Allen mentioned that his family has an extensive alcoholic background, and that his girlfriend
has substance abuse disorders running rampant through her family. He is aware that these diseases are
partially genetic, and that this poses an issue. They are both worried about the possibilities of their
children having substance abuse disorders as well. He does not want his children to live through the
same experiences he did during his active addiction. Allen has looked into programs for younger
children with addict parents. He hopes to get his children involved in these programs to lower the
chances of them using drugs and alcohol.

Mental Health History
The client was diagnosed with attention deficit disorder at fourteen years old. He was prescribed
Straterra to ease his symptoms of this disorder. He was also prescribed Wellbutrin around the same time
for clinical depression. The client sought treatment for sleep issues he was experiencing in 2008, and he
was prescribed Ambien to help him fall asleep. Lastly, he was diagnosed with chemical
dependency/substance abuse disorder in 2009. He has been prescribed Suboxone, and he currently still
uses this medication daily to ease the symptoms. Also, he received inpatient treatment for his substance
abuse disorder on two separate occasions. The first lasted only nine days, and he relapsed immediately.
The second was for thirty-five days after being released from jail. He was able to remain sober for an
extended period of time after this treatment program. During this time, he went to Alcoholics
Anonymous meetings on a daily basis.
Military History
The client denies having any military history.
Financial History
The client recalls his family being financially stable throughout his childhood. Allen admits to
be financially irresponsible from eighteen to twenty-three years old, and that resulted in a poor credit
score that still follows him. He is the manager of a popular local pizzeria. The client says that he is able
to pay his bills on time and does pretty well financially. Although, the client recognizes that he spends
too much money, and that he has to work on being more frugal, he believes he has made tremendous
progress when it comes to finances. Allen stated that he has no problem spending money on something
he is interested in without a second thought, and he does think about these expenses adding up and
creating consequences for him. He would like to work on this issue in the near future.
Legal History
The client was arrested in 2006 for statutory rape. He explains the charges as being a product of

her overprotective parents who disliked him. He was seventeen years old at the time and she was
sixteen years old. Her parents called the police on him when they found out she was with him after she
was told that she would not be able to see him anymore. The police officers found the two of them
hooking up in his car next to the high school. The charge was dropped to a felony assault charge that
way he would not have to register as sex offender. Allen was arrested in 2010 for hijacking a car (his
step-father's car). He spent three weeks in jail for this offense, and he was court-ordered to spend a
minimum of one month in an inpatient rehabilitation center at the request of his mother. This particular
offense was dropped from a felony charge to a misdemeanor charge. The client was ticketed a
misdemeanor charge in 2009 for possession of a hypodermic syringe, which he failed to pay on time.
The charge occurred in Wayne County's jurisdiction, and they did not care to pick up my client when he
was arrested in Macomb County in 2010. Allen went years without paying this ticket, and he was not
able to get a valid driver's license during this time due to this charge still pending. Eventually, Allen
went to the Wayne County courthouse, paid the bail, and scheduled a court date for his hearing. He was
ordered to pay a fine and provide a urine sample for the court to prove he has been clean. He was able
to do both that day. The case and charge were dropped.

Immigration issues
The client denies having any immigration issues.
Religious History
The client was raised Roman Catholic, and he has decided he did not want to be a part of that
religion any longer. He now subscribes to non-denominational Christianity. Allen was turned off to the
Roman Catholic religion because of some of the customs and beliefs they believe to be important. He
said that he holds a fundamental belief in a higher power, in his case, God, but he did not share the
same beliefs regarding his higher power. Allen describes himself as being religious, although he does

not regularly attend church services. The client stated that attending church services does not make an
individual any more holy than an individual who does not. He does believe in God and the Devil, and
Heaven and Hell. The client has read the Bible all the way through at least one time, and he reads
passages from the book on occasion. He does believe most of the stories in the Bible to be the truth,
although he cannot say that for everything in the Bible. Allen reports that he has researched other
religions in the past because he likes to know as much as he possibly can about any subject, especially
one as crucial as religion.
Educational History
The client struggled to graduate high school. However, he says that he did exceptionally well at
test taking, and because of that he was given a few scholarships. Those scholarships were used to attend
Macomb Community College for one and half years. He was working towards getting his E.M.T
certifications, and he was only one semester away before his legal record was discovered and he was
kicked out of the program. He never returned to an accredited higher education institution ever since.
Allen has enrolled in an online series of classes to prepare and take the proper test to become certified
for certain computer skills. He took specialized classes to become S.A.F.E certified to manage the
pizzeria. He does not have any desire to further his education. The client stated that his girlfriend does
try and push him to go back to college or finish the computer certifications often. The client believes
that he is very intelligent, but he admits to having poor judgment. He does not think he used his
intelligence to the fullest advantage by dropping out of school, but he loves working at the pizzeria.
Employment History
The client had chores he was responsible for taking care of in his household as a child. Some of
these chores included cleaning the bathroom, taking out the garbage, cleaning his room, mowing the
lawn, and vacuuming. The younger siblings would pitch in when they were old enough. He recalls his
sister not having to do many of the chores, and his younger brother as well. Most of the household

chores were left to Allen and his eldest younger brother, Adam. The client started to fight with his
mother on a daily basis at the age of fourteen. This prompted him to get out of the house and get a job
at a nearby Coney Island. He wanted to get away from his mother for a few hours, and he wanted to
make his own money so he would not have to rely on his mother so much. He started as a bus boy at
the Coney Island making minimum wage, which at the time was $5.25 per hour. The client said that the
Coney Island did shady things such as telling him he needed to clock out when they closed the
restaurant at night, even though he would stay an hour or two to clean the restaurant after closing. He
said the manager was very hard on him, but since he was only fourteen years old, and this was his first
experience working, he assumed that is just the way it is. He said that looking back he sees how terrible
the working conditions were at that Coney Island and how illegal some of the things they did are. Allen
worked at the Coney Island for one and a half years before figuring out that this job was not like all of
the other jobs. The client credits his hard work ethic to having that boss be so mean and hard on him for
his first job. His second job was at his father's friend's business, Roma's Pizza. He worked there for
about two years before the business closed their doors for good. Next, he worked at Papa Romano's
Pizzeria for the first time for about three years. He became a manager at this location after a year of
working as an insider for the business. During the time of his employment, he started to use drugs more
frequently, and by the end of the two years he was unemployable. He eventually quit once the owners
sold the store, and he remained unemployed for the next one and a half years or so. Allen was offered a
career as an electrician by his girlfriend's mother's longtime friend. By this time, he was addicted to
heroin, and he only showed up to his scheduled shifts sporadically. After ten months, he eventually
stopped showing up to work altogether. He remained unemployed for six months before receiving his
next job offer. His girlfriend's mother had a client who managed the Taco Bell near their house and was
able to get Allen a job there. Allen worked there for a little under a year before he went to jail and was
unable to continue working there. After he finished his jail sentence and went to the court ordered

inpatient rehabilitation program, he waited about six months to get himself together before searching
for another job. Eventually he ended up at Papa Romano's Pizzeria again where he still continues to
work today.
Strengths
The client listed computer skills and natural intelligence as his talents. Allen mentioned
computer skills as being his number one strength as well. Other strengths the client believes he possess
are good people skills, following directions, writing letters/papers, hard work ethic, fact memorization,
and logical thinking. The client boasted about his charisma and natural ability to make friends easily.
He admits that this same charisma worked in his favor when he was using drugs to manipulate other
people to get what he wanted from them. It has also helped him talk his way out of legal problems.
Allen's mother went to college to become an English teacher so he had an advantage over his peers
when it came to writing letters or papers. Allen said that he is above average when it comes to writing
creatively, as well as grammar and spelling. When I asked what he meant by logical thinking being one
of his strengths, he said that meant reasoning. He gave me the example of taking a test in school and
not knowing the answer what-so-ever, but he has a unique ability to logically think about what the
correct answer is. He is even amazed at a few times when none of the answers appeared to be logical,
but he somehow still managed to choose the right answer. The client said that his biggest support
system is his girlfriend. Allen said that he can turn to certain members of his family for support too.
Some of those family members he is referring to would be his mother, father, step-father, and his uncle
Joe. Allen said that he has bonded with some of his co-workers that he has worked with for the past few
years.
As the social worker, I noticed some of the client's strengths during the interview. I agree with
client when he stated charisma was one of his strengths. He does having a way of drawing you in
during a conversation. The client portrayed positive people skills by using manners, making jokes,

making eye contact, and smiling. It appeared to me that he thought highly and lowly about himself at
the same time, which threw me for a loop. He was always at one extreme or another: extremely
confident or self-conscious. The client talked a lot about his long-term girlfriend, and when he did talk
about her he seemed to talk different and almost glow in a way. I would agree that she is his biggest
support system, although their relationship sounds like it is unhealthy in a co-dependent way. This is
one of the issues presented to me during the interview that I would like to form a plan of action to
improve. The main priority is the issue of substance abuse, which has to come first.

Step One
Work with the client.
There is a big difference between working WITH the client as a team and working AT/FOR the
client by not actively listening and not allowing or attending to the statements the client is making. The
client's input is critical for an effective session. The social worker generally should not talk nearly as
much as the client does in the interview. This is about the client, not the social worker. The client,
Allen Riddell, has sought my professional assistance with a number of issues he is having. Allen
described the issues as follows:

Drug and alcohol addiction

Low self-esteem

Co-dependency issues

Relationship issues

Step Two
Prioritize the client's issues.

1. Help get Allen off drugs and alcohol by providing resources, support, and therapy this is the

solution not the issue. The problem is that he uses drugs

The book is between $15-20, which can be expensive for a drug addict to afford.

He needs to be motivated to read the book.

Mezzo Strategy: Sign up for group therapy.


Pros:

It may be easier for the client to share openly with other addicts.

He may find friends who set a positive example for him.

He will see some success stories.

Group members tend to supportive of one another, which may increase his motivation and
confidence.

It will provide a social setting so he does not feel so isolated.

Cons:

He may not want to share openly with other people around.

He may see other addicts fail and lose hope.

He may make friends that are a negative influence which may lead to a relapse.

He may not get along with everyone in his group.

Macro Strategy: Inpatient Rehabilitation Treatment


Pros:

He is less likely to leave and relapse in a professional closed setting.

He may learn a lot during the mandatory classes.

He is in professionals hands who can properly monitor his detoxification.

He is surrounded by individuals who share common relatable issues with him.

He has a solid professional and peer support system.

Cons:

He will have to put his outside life on hold until he graduates from the treatment program.

It is costly to attend inpatient treatment.

It takes work and effort to finish the programs.

You have to live in dorm like conditions with no privacy, community restrooms/showers, and
eat cafeteria foods (for most middle income rehabs).

You have to follow the rules and do chores.

Need #2: Improve relationship in a healthy manner and reduce tension


Micro Strategy: Look into couple's therapy
Pros:

The therapist may work as a mediator to work out issues.

The therapist will prove a new insight of the situation from an outsider looking in on it.

They may both feel more comfortable discussing their feelings in this setting.

It may help them both actively listen to each other more.

Cons:
He may not agree with the therapist

He may think the therapist is picking sides, which might cause even more issues for the couple.
He may not respond well to couple's counseling.

Mezzo Strategy: Sign up for couple's retreat


Pros:

It may strengthen the couple's relationship.

The relaxing intimate environment will help ease tension and build affection.

The activities may help the couple get closer again.

The retreat may include professionals to assist them therapeutically.
