week to identify how he/she would organize care for a group of patients and demonstrating delegation that incorporates
interdisciplinary team systems thinking, and consideration of effective leadership plans.

Supportive Faculty Comments: Is able to prioritize herpatient assignment and is able to do so while working with all
members of the health care team. She leads by example and in a timely manner. She has grown and is much better with
using collaboration in advocating for her patients. She is becoming a transformational leader, in that she is a systems
thinker, delegating appropriately, and learning to empower those around her.

Evaluate a plan of coordinated care for diverse populations across the lifespan who
have a varietyof health states in acute care, long term care and community settings.

determine effectiveness/appropriateness of proposed plans of care. (1-2 day pull out to complete on-site).

Supportive Faculty Comments:Participates in our Interdisciplinary Rounding, working on establishing diverse plans of
care to meet the needs of our diverse patient population. Has gained more understanding that our diverse population has
more to do with financial and educational background as where someone comes from. Most patients in this clinical setting
are acutely ill, but do share some common diagnosis. Has gotten much better with experience and confidence is growing!

safe standard of care, advocacy for patients, and an improved health care
environment (Health Care Environment)

presentation to peers/instructor in post conference.

Supportive Faculty Comments: Is always using clinical reasoning and sound ethical judgment in providing patient care
and working with their families. She is always safe with patient care and is much more comfortable in advocating for the
patients she cares for. Is able to process making plans for patients and their families and in communicating these plans with
her patients. Has proven she has integrity and is honest in all aspects of her practice. She has also grown in her critical
thinking and is putting proactive plans in place to keep her patients safe.

Supportive Faculty Comments:Is always on time, prepared and looks professional. She is pleasant, cooperative and always
willing to learn. She is the first one to offer help and is getting better with anticipation\intuition in identifying and acting on
the needs around her. She never takes a frustrating day into her patients rooms and is very respectful of everyone she
comes into contact with.
