Grade Five Bowling Field Trip

Yes, I will allow my child to attend the bowling field trip to Dubai Bowling
Center on graduation day, Monday, June 15. I am submitting 100 dirhams
with this signed form which will cover the approximated cost of a graduation
hat, transportation to and from the bowling center, two games of bowling, as
well as lunch. (We will leave the school by 10:30 and return before 2:00.)

No, my child will be unable to attend. (If no, 20 dirhams is kindly requested
to cover the cost of the graduation hat.)
