What is
Communication?

Communication, in essence, is the exchange of

information between individuals in a shared and
understood language through a common system.

merriam-webster.com

In nursing, we communicate through

Documentation: written statements that are relied upon as
proof of services given and received, patient activities and
actions
Records: documentation of information relevant to the
individuals health care.

(Potter & Perry, 2013)

Records
Records are valuable sources of current and
accurate information about a patients health care
status (Potter & Perry, 2013).

What is the purpose for Medical

Records?

Medical Records Purpose

Source of legal documentation in the court of law
care not documented is care not provide
Documents of supplies and equipment used for financial
billing/reimbursement.
MR used for education to learn the nature of an illness
MR used for research studies and provide statistical data.
For auditing/monitoring staffs development
(e.g. answer if all standard of care are met and if there certain
areas that need to be improved?)
Potter & Perry, 2013

Documentation

Organized

Methods of Documentation
Paper and electronic health records: patients health record
information
Narrative: Story-like format
Problem-oriented medical record: focuses on patients
problems and coordinating a common plan of care.
We must document!
As we have come to hear and know very well: If its not
documented, it wasnt done!

So what does all of that have to do with nursing

informatics?
Were glad you asked
We have an answer for you, but first, what is
nursing informatics anyway?

Nursing Informatics
Is the integration of nursing science,
computer science and information science
to manage and communicate data,
information, and knowledge to support
patients, nurses, and other providers in
decision making in all roles and setting
(Perry & Potter, p. 359).

Informatics Legislative Background:

Obama promised to have every citizens medical records stored digitally
EHR will improve quality of care, manage care cost and prevent medical
errors.
2009: Congress passed, the American Recovery and Reinvestment Act
(ARRA)

$19 billion would be funded to hospitals and providers that

showed meaningful use of EHR
This set forth the EHR implementation projects in clinical
settings.
(Murphy, 2010)

How is informatics used?

IV Pumps

Barcode Technology
Electronic Medical Record
Yes! Facebook and twitter

IV Pumps
Improve patient safety by avoiding adverse drug events (Breland, 2010).
According to a study conducted by Adochi and Lodolace (2005):
i.v. pump-related wrong-dose errors fell from 24 in 2002 to 10 in 2003
after implementation of smart pump technology in January 2003 in a
hospital located in California
Another study revealed infusion rate errors were reduced down to 4 in a 16
month period from 15 incidents the prior 16 months (Fanikos et al., 2007).
IV medication errors were reduced by 73% in a pediatric hospital after the
implementation of IV pumps (Lason et al., 2005)

Bar-code Medication System

Use of the bar-code eMAR substantially reduced the rate of
errors in order transcription and in medication administration as
well as potential adverse drug events, although it did not
eliminate such errors.

(Poon et al., 2010)

How many of you know what this is?

EMR is the information that is collected in a healthcare setting at a particular

time and place (Potter & Perry, 2013).
EMR could be very effecting in promoting health and disease prevention if
used to their fullest potential and helps in avoiding making mistakes (Furst et
al., 2013).
Electronic care plans help promote collaborative team approach (Furst et al.,
2013).

Impact of Informatics on Nursing

Education
The Technology Informatics Guiding Education Reform (TIGER) Initiative is designed to address a set of skills
that is needed by all nurses who will practice in the profession in the 21st century (Hebda & Calderone, 2010).
TIGER recommends nursing informatics as a core competency for health care professionals (Nursing
Perspectives, 2008).
A study was conducted by the NLN to see how informatics was being integrated into the educational system.
BSN and higher degree nurses had the most integration (Nursing Perspectives, 2008).
Another study done by Choi (2012) further describes the distribution of the integration, knowledge and skills of
BSN into RN to BSN programs, Accelerated BSN track and the Traditional BSN program. The scores indicated
that RN to BSN and the Accelerated BSN are more competent, but not the Traditional BSN students.

Advantages
Supports health care providers in their routine task and facilitate
preventive medicine
EMR system will prompt needed test, procedures, or consultants to
monitor patients condition.
Prevent medical errors and improve patient safety
Improve medical documentation
Automatic data collection saves time for nurse
Promote collaborative care
Support clinical decision
(Ericksen, 2009; Furst, Finto, Malouf-Todaro, Orr, Santos, Sutton & Tipton, 2013)

Disadvantages
HIPAA compliance and patient privacy concerns
Informatics will complicate daily routines
nurses need to focus on patients and not be burden by new
technology
Templates discourages complete and detail documentation which is useful
in patient care decisions.
WHAT ARE SOME DISADVANTAGE YOU SEE IN NURSING
INFORMATICS???
(Ericksen, 2009; Furst, Finto, Malouf-Todaro, Orr, Santos, Sutton & Tipton, 2013

Future of Nursing Informatics

The future holdhttps://www.youtube.com/watch?
v=YoZ298ytpVEs more excitement and promises
NO need for Face to Face OFFICE VISITS??
Cameras on patients home computers, will allow nurses to
do wound checks through transfer of images.
