Pt. was evaluated to fulfill requirement for class assignment.


Vocal Function Analysis

Perceptual: Pts voice perceived with mild dysphonia characterized by roughness. Pts. voice
perceived as normal in regards to durational control and loudness.

Frequency Evaluation: Pt. has normal fundamental frequency (132 Hz), normal frequency
variability (30.9 Hz), and normal phonational range (36 semitones).

Intensity: Pts. mean speaking intensity is low normal (66dB -1 ft. mouth-mic-distance).

Quality/Perturbation: Sustained vowel /a/ production showed increased jitter (1.19%); normal
shimmer (0.48 dB); and low normal HNR (20.04). Increased jitter and low-normal HNR are
consistent with perceived roughness.

Durational Measures: Pt. shows normal vital capacity (4.38 L) for his height (Normal-4.4L, 96%
of expected), normal MPT (21.81s, 75% observed/expected ratio), normal Phonation Quotient
(PQ=200 mL/s). Pt. was observed to use efficient diaphragmatic breathing throughout this
evaluation.

Impression: Mild dysphonia characterized by rough vocal quality and increased shimmer.

Recommendations: Voice therapy is not recommended at this time due to pts. ability to
effectively communicate.

Pt. was evaluated as requirement for class assignment. Pt. reported congestion due to a cold at
the time of evaluation.


Vocal Function Analysis

Perceptual: Pts. voice perceived with mild dysphonia characterized by strain. Pts. voice
perceived as normal in regards to durational control and loudness.


Frequency Evaluation: Pt. has normal fundamental frequency (119 Hz), reduced frequency
variability (16.7 Hz), and normal phonational range (38 semitones) .

Intensity: Pt. has normal mean speaking intensity (67 dB- 1ft mouth-microphone-distance).

Quality/Perturbation: Sustained /a/ vowel production showed increased jitter (.813%), normal
shimmer (.374 dB), and reduced HNR (17.902). Increased jitter and reduced HNR are consistent
with perception of mild roughness.

Durational Measures: Pt. shows normal vital capacity (4.02 L) for his height (Normal-4.50 L,
90% of expected), normal MPT (21.45s, 80% observed/expected ratio), normal Phonation
Quotient (PQ=187.4 mL/s). Pt. was observed to use efficient diaphragmatic breathing
throughout this evaluation.

Impression: Mild dysphonia characterized by strained vocal quality and reduced pitch variability,
may be due to congestion.

Recommendations: Voice therapy is not recommended at this time due to pts. ability to
effectively communicate.

Frequency Evaluation: Pt has normal speaking fundamental frequency (184 Hz), normal
frequency variability (27 Hz), and normal phonational range (41 semitones).

Intensity: Mean speaking intensity and range in reference to a1 ft. mouth-to-mic distance).
Pt has low-normal mean speaking intensity (65dB -1 ft. mouth-mic-distance).

Quality/Perturbation: Sustained vowel /a/ production showed normal jitter (0.394%), normal
shimmer (0.101 dB), normal HNR (22.67).

Durational Measures: Pt. shows low normal vital capacity (2.84 L) for her height (Normal-3.30L,
85% of expected), normal MPT (20.04s, 120% observed/expected ratio), normal Phonation
Quotient (PQ=141.7 mL/s). Pt. was observed to use efficient diaphragmatic breathing
throughout this evaluation.

Impression: Perceptual, acoustic, and durational measures indicate normal voice.


Recommendations: Voice therapy is not recommended at this time.

variability (37 Hz). Pt. exhibits normal phonational range (32 semitones).

Intensity: Pts. mean speaking intensity is normal (65 dB- 1ft mouth-microphone-distance).

Quality/Perturbation: Sustained vowel /a/ production showed normal jitter (0.479%), normal
shimmer (0.269 dB), and normal HNR (25.22). Normal perturbation measures are consistent
with perceived normal voice.

Durational Measures: Pt. shows normal vital capacity (3.43 L) for her height (Normal-3.15L,
108% of expected), normal MPT (22.35s, 110% observed/expected ratio), normal Phonation
Quotient (PQ=153.5 mL/s). Pt. was observed to use efficient diaphragmatic breathing
throughout this evaluation.

Impression: Perceptual, acoustic, and durational measures indicate normal voice.


Recommendations: Voice therapy is not recommended at this time.

Intensity: Mean speaking intensity and range in reference to a1 ft. mouth-to-mic distance).
Pt. has low-normal mean speaking intensity (65dB -1 ft. mouth-mic-distance).

Quality/Perturbation: Sustained vowel /a/ production showed normal jitter (0.25%), normal
shimmer (0.069 dB), and normal HNR (31.08). Normal perturbation measures are consistent
with perceived normal voice.

Durational Measures: Pt. shows low normal vital capacity (3.32 L) for her height (Normal-3.50L,
94% of expected), normal MPT (30.88s, 150% observed/expected ratio), normal Phonation
Quotient (PQ=107.5 mL/s). Pt. was observed to use efficient diaphragmatic breathing
throughout this evaluation.

Impression: Pts. presents with low fundamental frequency; pt. able to effectively communicate,
no other signs of disordered voice.


Recommendations: Voice therapy is not recommended at this time.
