Senior Veterans Council (SVC) Pricing Structure

Fully-Developed Claim with Resource Pre-Planning Consultations

Widow: $1,200

Veteran: $1,500

Senior Veterans Council Agents and staff, in consultation with SVC retained appropriate professionals
(including Registered Nurse, Geriatric Care Manager; Certified Medicaid Planner and Elder Law Medicaid
Certified Attorney), will provide the following consultations and services for Care and Resource Planning
and Qualification of Benefits.

Services for Veteran or Widow:


Half-hour consultation with SVC retained Elder Law &


Medicaid Certified Attorney

Presentation and assistance in preparation of When


a Loved One Dies Beneficiary Guide Book

Two-hour consultation with clients retained attorneys


and financial/tax advisor

Advice & guidance on home health care and facility


care options (facility placement consultation)

One hour consultation with SVC retained Licensed


Geriatric Care Manager

Review by SVC retained Elder Law attorney of


existing POA to assure the provisions to allow for
worst-case end-of-life and legacy planning (1)

Development of the care assessment and preplanning of physician statements by SVC Registered
Nurse to asses and establish ADL needs

Pre-planning and coordination of current and


potential VA claims to assure a fully developed, fast
track submission (1)

Use of care contracts for at-home care, and/or facility


statements to meet VA FAST Letter requirements

Advice on positioning or conversion of assets


necessary of benefit qualification (and attendant
professional tax advice) from SVC retained attorney

Consultation regarding Medicare rules on suspension


of Post Hospital Rehabilitation benefits and
assistance in appeals process

Strategies for assuring that the family has access to


potential accrued benefit should the claimant die prior
to the award (1)

Guidance and assistance in in obtaining commercial


and/or community-based Home Health Care services

Consultation with CMP (2) on accessing Medicaid via


Spousal Asset Allowance Rule and use of SPIAs/
Promissory Notes/Personal Services Care Contracts
to preserve 100% of current assets

Consultation with CMP (2) and/or SVC Medicaid


Attorney to understand qualification strategies and
Medicaid Applications and Appeals process

Advice on obtaining or restoring VA benefits while


claimant is at home, utilizing family care contracts

Discussion of strategies to address vacating, selling


or renting the claimants home and to prevent loss of
future claims eligibility

Consultation with CMP (2) on strategies for


dovetailing VA claim with potential Medicaid and
other VA, government, & community benefits
Consultation with CMP (2) on strategies concerning
Medicaid regulations (including half-loaf ) to
preserves assets while securing Medicaid Eligibility

centralized location to collect, review, cross check
and compile claims in signature ready format (1)

Tracking of VA claims by designated VA agent via

* Note (1): There is no charge for assisting with the actual preparation and submission of any possible future
claims or following at VA Pension Office if Claimant is a planning client of SVC office and we are appointed as VA
POA.

Pro Bono (Free) Service:

Claimant/Family is provided VA forms and VA instructions to submit completed claim to the VA. SVC staff
is available to answer general questions, but does not serve as VA Appointed Agent and therefore will
provide only general advice and guidance prior to submission. SVC will not serve as Agent for following
claim at the VA.
VA Toll Free Number, (800) 827-1000, is available for claimant to contact VA for specific benefit questions
and status of claim after filing.
