affirm as under:i.

I am a subscriber of Telenor network using Telephone No 03335320314.

ii.

I had used my above stated telephone number for generation of bulk SMS
communication beyond the limit as defined by competent Authority. As a
result of the above, outgoing SMS facility of my subscription has been
suspended.

iii.

I hereby undertake not to cross the SMS outgoing limit in future as

defined by the Authority from time to time.

iv.

That whatever has been stated above is true and correct to the best of my
knowledge and belief.

In view of the undertaking given herein, I request that outgoing SMS facility of my

telephone subscription may kindly be restored as I pledge to use my telephone number according
to rules and regulations set by PTA.
