So: Are you okay? Where is the pain located? Let me call the nurse.
Nurse: Where is the pain located? Can you rate the pain from 1 to 10

10 is the highest?
Pt: 10.. Its really painful.
Nurse: Okay let me call the doctor.
Dr: Prepare the patient. Were going to have a stat appendectomy.

Ward nurse endorses the pt to the holding nurse inside the OR.
Make sure all the forms are completely filled up such as the
Pre-operative Information sheet (PIS), Pre-operative checklist, CP
clearance and other necessary forms prior to the surgery.

9. Scrubbing
What is scrub? It is to wash the hands and forearms thoroughly, as for
surgery. To scrub in implies the use of a brush.

Gown, gloves, mask, bonnet and sterile behavior

Keep hands in sight or above waist level away from the face.
Arms should never be folded.
Articles dropped below waist level are discarded.
Drying
Dry one hand and arm, starting with the hand and ending at the
elbow, with
one
end of the towel.
Dry the other hand and arm with the opposite end of the towel.
Drop the towel.
Gowning
Pick up the gown. Apply a pincer grip and be careful not to touch
anything on the sterile field.

Move away from the trolley and start unfolding the gown.
Do not shake or twist the gown. Rationale: It can cause turbulence
resulting to debris and other particles to rise from the
floor causing
contamination.
Locate the sleeves. Always be aware of your surroundings.
Another person (circulatory nurse) who is not scrubbed will pull
the
gown onto you as you extend your hands through the
gown cuffs.

Principle of sterile technique
#1 Only sterile items are used within the sterile field.
All articles used in an operation have been sterilized previously.
If in doubt about the sterility of anything consider it not sterile. If a
non-sterile person brushes close consider yourself
contaminated.
Supplies to be used for the surgery are prepared. The circulating
nurse opens the wrapper for the sterile packages.
13. Gloving
Request for your preferred glove size and type.
Use the enclosed left hand to turn the glove upward facing the
right
hand.
Open the cuff of the glove with the left hand and slide fingertips of
the
right hand into the cuff.
Take the double layer of the glove on top of the right hand and
pull it
over to
the back of the right fist and thumb.
Take the rim of the glove and pull it over to the wrist. Pull the
sleeve of
the
gown, allowing the fingertips to emerge until the
glove is in the
correct
position.
Repeat the same steps for the right hand.
#2 Persons who are sterile touch only sterile articles; persons who
are not sterile touch only unsterile articles.
14. Offering the gloves to Norman then gowning
#3 Gowns are considered sterile only from the waist to shoulder
level in front and the sleeves to 2 inches above the elbows.
Pick up a gown from the sterile linen pack. Step back from the sterile field and
let
the gown unfold in front of you. Hold the gown at the shoulder seams with
the gown
sleeves facing you.
Offer the gown to the surgeon. Once the surgeons arms are in the sleeves, let
go
of the gown. Be careful not to touch anything but the sterile gown. The
circulatory
nurse will tie the gown.
Pick up the right glove. With the thumb of the glove facing the surgeon, place
your fingers and thumbs of both hands in the cuff of the glove and stretch it
outward, making a circle of the cuff. Offer the glove to the surgeon. Be careful
that the
surgeons bare hand does not touch your gloved hands.

Repeat the preceding step for the left glove.

#4 Sterile persons keep contact with sterile areas through the use of sterile gloves and gown.
15. Patient in the OR table
#5 The patient is the center of the sterile field. The sterile field includes the OR table, tables
covered with sterile drapes and persons wearing sterile gowns and gloves.

#6 Only the top of the sterile drape is considered sterile.
Anything falling or extending over the edge of the table is considered unsterile. The scrub
nurse does not touch the part of the sterile drape hanging below the table
Level.
#7 Sterile persons avoid leaning over an unsterile area; non-sterile persons avoid
reaching over a sterile field. Unsterile persons do not get closer than 12 inches
sterile field.
#8 Edges of anything that encloses sterile contents are considered unsterile.
#9 Sterile areas are continuously kept in view.
#10 Sterile persons keep contact with sterile areas to a minimum.
