WHAT IS A
PERSONAL
HEALTH RECORD?

The personal health record (PHR) is


an electronic, lifelong resource of
health information needed by
individuals to make health decisions.

The personal health record (PHR) is


an electronic application used by
patients to maintain and manage
their health information in a private,
secure, and confidential environment.

A personal health record is generally understood to


be a collection of information about an individual's
health or health care services, such as
hospitalizations, doctor visits, medications
(prescriptions and over-the-counter drugs), allergies
and other medical conditions that can be stored on
the individual's behalf for as long as he or she
chooses.

Is a universally accessible, layperson


comprehensible lifelong tool for managing
health information, promoting health
maintenance and assisting with chronic
disease management via an interactive,
common data set of electronic health
information and e-health tools.
