1. Registration entitles children 12 and under to:

2. Each form allows a maximum of four (4) children to be registered, who MUST
be accompanied by a parent/guardian at the event. Parent/guardian should be no
less than 18 years old.
3. Forms must be signed by the accompanying parent/guardian who will be asked
to present a valid form of identification on the day of the event.

4. Registration is FREE for the first 300 children, thereafter the registration fee
shall be $20. per child.

7. All registration items must be collected by the accompanying parent/guardian

named on the form by 3pm on the day of the event. Failure to do so will result in
forfeiture of the registration.

8. By registering, you agree to all the terms and conditions listed above and further
agree to abide by all instructions and guidelines given on the day of the event.
Additionally, I have no objection for the organizers to use any photos and/or videos
of me and the children in my care for publicity purposes and that I will undertake
to indemnify the organizers, sponsors and/or any other unmentioned related parties
against all liabilities for death, personal injury (either fatal or otherwise), loss or
damage to property and/or any losses, damage, cost and expense whatsoever and
howsoever caused in the duration of my participation in Fundangos Funtastic
Fiesta.
