Parents/Guardians, Chaperones and Students,
Please use this checklist to make sure you have all required paperwork turned in to
Mrs. Zabala no later than Friday, March 18. Without this paperwork filled out in its
entirety your student and/or chaperone will not be able to attend the trip.

Medical insurance is MANDATORY in order to attend the trip
A copy of the medical insurance card (front and back) MUST
be provided

At School Coverage (with dental):
HIGH OPTION--$37.00
LOW OPTION--$29.00
Coverage is effective IMMEDIATELY and lasts through the end
of the school year

Retain a copy of the Trip Itinerary and Student Information Packet

Please note: Students MUST be picked up by 7:00pm or

proper authorities will be called to sit with your child.
Teachers and/or chaperones WILL NOT wait for any parents
not at the school by 7:00pm.

Each room will have 4 students, all of the same gender. These students will all be in
the same group with the same chaperone. Students may choose their roommates,
however, Mrs. Zabala may need to assign and/or split up roommates as needed
(behavior, etc.).
Chaperones have 2 people per room, of the same gender unless relatives are
chaperoning (ex: mom & dad are both chaperoning).
Students MAY NOT room with adultsno exceptions.
