PRACTICE BEHAVIOR 8.1
Critically choose and implement interventions to achieve practice goals and enhance capacities of clients and constituencies. I was able to use this practice behavior by mentioning some possible ways of solving patient dilemmas. Patients were able to depict which intervention path they wanted to go, and the supervisor and I were able to assist them to see it through.
PRACTICE BEHAVIOR 8.2
Apply knowledge of human behavior and the social environment, person-in-environment, and other multidisciplinary theoretical frameworks in interventions with clients and constituencies. I was able to complete this practice behavior by applying what I have learned in class and through research and apply it to each individual patient situation. For example the developmental framework helped me focus on the ways the body changes over a period of time. Most patients cannot do as they once did.
PRACTICE BEHAVIOR 8.3
Use inter-professional collaboration as appropriate to achieve beneficial practice outcomes. I was able to use this practice behavior by using what I have learned in class to apply to patients. Using role playing in class was a great way to assist me to know which questions to ask and how to ask them to patients. Some patients are more difficult than others, depending on their level of pain and diagnosis. For example, a patient with Alzheimer's can not be asked a question in the same way I would ask someone with no dementia at all.
PRACTICE BEHAVIOR 8.4
Negotiate, mediate, and advocate with and on behalf of diverse clients and constituencies. I used this practice behavior by allowing the patient to have some options. There goals of returning home may be different from that of what the professional thinks is good for them. As professionals we have to honor and respect the patients wishes, no matter what they choose to do.
PRACTICE BEHAVIOR 8.5
Facilitate effective transitions and endings that advance mutually agreed-on goals. I used this practice behavior by assisting the patients fill out forms, filling out applications for the patient, and setting up care plan assessments to discuss options with the patient and patients family, therapist, social worker, and nurse.
HOW I USED THIS COMPETENCY
I have used this core competency by leaving the decision making up to the patient, but giving them options and discussing pros and cons of each option. I have used critical thinking to depict which options would be best available for each patient. I have also helped by setting up care plan meetings for everyone to come together to decide where and when the patient should discharge from the facility. These meetings are for the patients best interest.
