General Agreement

This agreement is made between DMMSA and

______________________________________.

_______________________________ will be participating as an artist at the “Catch the

Vision” art exhibition. She/He/They will be responsible for the $5 artists’ fee on or before May
20th. (Please make checks payable to DMMSA) They must be present at registration at 8am
Saturday June 5th and 8am Sunday June 6th, unless they have chosen to only exhibit one of the
two days. They need not be present at 8am either day if they are performing on the stage in the
band shell, then they will need to register at least one (1) hour before they are to perform. They
will also be responsible for outfitting their space, i.e. tables, chairs, props, etc. Any performances
or displays utilizing pornographic images/imagery or foul language will be asked to leave. By
participating in any events organized by DMMSA, you agree to the following: DMMSA events
are provided with no special warranty either express or implied. The Organizers, including but
not limited to the Founders, Committee Members, Advisory Board Members, Sponsors, etc., of
DMMSA events assume no liability for any loss, theft, damage, or injury to property or persons
whether arising in contract, negligence, equity, or otherwise. Participation in DMMSA events is
considered "at your own risk". All participants must use due care under the circumstances to
comply with all rules, regulations, statutes, and laws of the State of Iowa.
