I. Setting:
( Describe the population
and special learning circumstances)

Year: 2015-2016
Date: October 26, 2015

I teach 18 first grade students in all subject areas. A large percentage of

my students are eligible to receive free or reduced lunch.
All of my students will participate in whole group math. They will also
work in differentiated small groups with the teacher, and work in math
stations needed for remediation.

IV. Goal Statement

(Describe what you want the
learners to accomplish )

In my classroom, upon giving the 15- problemincluding various math

concepts, scores ranged from 0 to 11. Out of 18 students:
8 scored 0 to 5 (Intensive intervention)
10 scored 6 to 11 (Strategic intervention)
0 scored 12 to 13 (On level)
By the end of the 2015-2016 school year, 100% of my students will make
measureable growth on the McGraw Hill Math Placement Test. At the end of
the year, measureable growth will be defined as:
Fall Student Performance Level
Intensive Intervention (0-5 correct responses)
Strategic Intervention (6-11 correct responses)
On-Level (12-13 correct responses)

Spring Student Per

Strategic Intervention (6-11
On-Level (12-13 correct res
Above-Level (14-15 correct

At mid-year, students that scored at the intensive intervention

performance level will increase their scores to the strategic intervention
performance level on the McGraw Hill Math Placement test. Students
that scored at the strategic intervention performance level will increase
their scores to the on-level performance level on the McGraw Hill
Placement Test. Students that scored at the on-level performance level
will increase their scores to the above-level performance level.
By the end of the school year, 100% of students will score at the above
level performance level.
