CONSENT FORM

You are invited to take part in a research study designed to better understand elementary
general education teachers as they teach the Common Core State Standards (CCSS) using
kinesthetic movement. The researcher is inviting experienced teachers with at least 2 years of
teaching experience in general elementary education who use kinesthetic learning plans as part
of a purposeful sample to be in the study. This form is part of a process called informed
consent to allow you to understand this study before deciding whether to take part.
This study is being conducted by a researcher named Heidi C. Erickson, who is a doctoral
student at Walden University.
Background Information:
The purpose of this study is to gain insight into the lived experiences and perceptions of
elementary education teachers who teach Common Core using kinesthetic movement, as an
innovative teaching strategy. In particular, I am interested in hearing their ideas and
experiences and in teaching strategies as they use movement for instructing their students in
the Common Core. The research questions that guided this study were developed based on
concerns about the decreasing opportunities that students have to physically move within typical
school day.
Procedures:
If you agree to be in this study, you will be asked to:
Preview some basic information about the study.
Fill out a consent to participate and I will contact you within two days.
Teach one resource lesson plan using kinesthetic movement, known as kinesthetic
learning plans (KLPs). You will have up to two weeks to teach the lesson. When you
have completed the lesson, please email me at heidi.erickson@waldenu.edu.
Complete a written reflection immediately after teaching the lesson directly on the KLP.
Be interviewed and share your lived experiences, feelings, and perceptions about using
the KLP in one sixty-minute interview. The interview can occur through a phone call, in
person, or through a skype call. It will take sixty minutes and I will be asking you some
questions in a semi-structured interview format. These interviews will take place in a
three-week window and at your convenience. If in person, you can choose the place
and time. If a personal phone call or skype call is desired, we can meet at an agreed
upon time to carry out the interview. When the interview is completed, you will be asked
to scan and email your KLP along with your reflection to heidi.erickson@waldenu.edu. I
will notify you of the time window for your convenience.
Participate in a subsequent thirty-minute follow-up interview for clarification as needed.
This will be in the same format as the initial interview with time and place suggested by
participant. This will occur within a two-week period. I will notify you of the time frame
for your convenience.
Participate in member checking. Member checking is where you will be asked to verify
the data and check for accuracy. This will occur within a two-week window so that you
can be sure of my interpretation of the data and correct any misunderstanding or errors
on my part. I will contact you for your consideration of a convenient time frame. This will
occur by emailing the transcripts for your perusal with a follow-up phone call, skype or in
person.
Here are some sample open-ended questions:

1. Please talk a little about your current teaching position.


2. Please share a little about which Kinesthetic Learning Plan (KLP) you chose and why
you chose that one.
3. How much time to you spend planning to teach the KLP?
4. Please tell me about how the KLP was introduced to students? (instructions,
expectations)
5. Please tell me a little about how you taught the KLP (cues, directions, student roles).
6. Within the context of learning, can you please share some of your feelings about how
student were learning and how did that make you feel. You may use you reflection to
refresh your memory.
7. Please talk about what you saw the students doing during the KLP.
8. Please tell me about what you heard the students discussing during the KLP
Voluntary Nature of the Study:
This study is voluntary. Everyone will respect your decision of whether or not you choose to be
in the study. No one at any organization or institution will treat you differently if you decide not
to be in the study. If you decide to join the study now, you can still change your mind later. You
may stop at any time.
Risks and Benefits of Being in the Study:
Being in this type of study involves some risk of the minor discomforts that can be encountered
in daily life, such as anxiety about teaching performance, stress of learning and teaching a new
teaching strategy, finding time to teach the lesson as directed. Being in this study would not
pose risk to your safety or wellbeing.
Acknowledgments

Payment:
There will be no form of payment or compensation for participants in this study.
Privacy:
Any information you provide will be kept confidential. The researcher will not use your personal
information for any purposes outside of this research project. Also, the researcher will not
include your name or anything else that could identify you in the study reports. Data will be kept
secure by being placed on an external drive and placed into a locked filing cabinet. Data will be
kept for a period of at least 5 years, as required by the university.
