W. 12th Ave., Rm 202, or by email (JPEGs preferred) to mosaic@mosaicosu.com. By submitting this form, you are volunteering
your work for reproduction at the discretion of Mosaic Magazine.

How did you hear about Mosaic?
Please include 3-4 sentences about your work or yourself. If chosen, this will appear at the Art Show and may appear in the 2009 issue
of Mosaic. Please print legibly. If you’d like to include a mission statement, attach it to this form upon submission.

Submission Guidelines (please read carefully)

General:
• Artists must be undergraduates at The Ohio State University.
• All submissions must be slides, photographs, or scans of artwork; no original work will be accepted.
• No more than 3 submissions per artist.
• Works in a series may be counted as one entry, but must be labeled so, detailing the order of progression.

• All submissions will be judged anonymously and will be considered for the Albert J. Kuhn Awards, publication in 2009
Mosaic, and presentation at the unveiling ceremony.

• Works and slides must be retrieved at Kuhn Honors & Scholars House after the Art Show of Spring 2009. Mosaic is not
responsible for works unclaimed beyond this deadline.
Slides/Photographs/Scans:

• Must have the artist’s name, the work’s title, the piece’s actual-size dimensions, media, and an indication of which
direction is up. Specify number order if part of a series. If the work is untitled, please specify this.
• When submitting slides/photos of 3-D work, limit the number of slides/photos depicting different views to three.

• CD-ROM is accepted, but must have all information listed above for each frame. Email submissions are accepted. All
images sent by email must be no more than 1000 pixels in any direction, and compressed to the highest JPEG quality.
• Remember: if your piece is selected, we will need the actual work for gallery display during Spring Quarter 2009, in which
case your work must be ready-to-install for display.
All artists submitting work will be notified of decisions by late March 2009.
