The facility’s Resident Assessment Instrument (RAI) process is coordinated by a Registered Nurse (RN)
and in accordance with federal regulations.

Summary of Duties:
A Registered Nurse coordinates the completion of the RAI with the facility’s Interdisciplinary Team
(IDT). An RN serves as the Minimum Data Set Department Manager. The RAI Coordinator facilitates
the completion of Omnibus Reconciliation Act (OBRA) and Prospective Payment System (PPS)
assessments. [MDS Completion, Transmission, Validation Policy]

Physical Requirements:

You must be able to move intermittently throughout the work day. See, hear, or use prosthetic devices to
enable use of the senses adequately. You must be able to assist with the evacuation of residents.
Bending and stooping, sitting for extended periods of time, dexterity of hands and fingers to operate
equipment; and the ability to lift, move, push or pull a minimum of fifty (50) pounds are requirements of
this position.

Work Environment:

Your primary work environment is throughout the facility and at times you may work under stress. You
may be exposed to blood, body fluids, infectious diseases, and air contaminants. You may be subject to
work call-back during times of facility emergency or disaster preparedness functions.

Qualifications:

The RAI Coordinator will possess a current, unencumbered, active license to practice as a Registered
Nurse in this state. Must be able to read, write, and speak English. Must have 3 years’ experience as an
RN. Must have one year experience with MDS completion and one year of supervisory experience. Must
be able to work effectively with patients/residents, co-workers, and support agencies. Must possess good
communication skills in addition to interpersonal relations that will facilitate effective interaction with
staff.

Must be mature with integrity to manage the confidential aspects of the facility and to establish a working
relationship with residents, families, sponsors, and staff. Must implement good judgment with fairness
and consistency toward staff when making decisions.

facility management.

RAI Coordinator-Job Description

Essential Duties:

1. Will schedule the ARD’s (Assessment Reference Date) for Payment Assessments with the Rehab
Director daily, weekly and as needed in a manner that accurately captures the RUG (Resource
Utilization Group) category.
2. Will provide a schedule of ARD’s and Assessment types weekly and as needed to the IDT in
order to facilitate the timely completion of MDS sections and CAA’s (Care Area Assessments) by
each discipline.
3. Will schedule all OBRA (Omnibus Budget Reconciliation Act) ARD’s daily, monthly and PRN.
The schedule will be provided to the IDT. The RAI Coordinator will schedule Admission,
Quarterly and Annual Assessments. Addendums to the schedule will be provided by the RAI
Coordinator to the IDT as needed as it relates to Discharge Assessments and SCSA (Significant
Change in Status Assessments).
4. Is expected to use the RAI Manual as a resource during the assessment coding process.
5. Will coordinate the completion of MDS sections according to facility assignments.
6. Will communicate to members of the IDT as needed based on timeliness of completion. The RAI
Coordinator is expected to report any issues with timely completion to the Administrator
immediately.
7. Will facilitate with the IDT members, the completion of assessments and corresponding due dates
for Z0400A-L, Z0500B, V0200B2 and V0200C2 according to the RAI Manual; reference
Chapter 2, Section 2.6, Page 2-15
8. Will ensure the transmission of PPS and Quarterly Assessments within 14 days of the
completion date in Z0500B; reference Chapter 5, Section 5.2, Pages 5-2 and 5-3
9. Will ensure the transmission of Comprehensive assessments within 14 days of the Care Plan
Completion Date (V0200C2). See Assessment Typing document.
10. Will confirm the transmission file and review the printed initial and final validation report.
11. Will facilitate the correction of any fatal errors immediately and retransmit the assessment until
an accepted validation report is received.
12. Will also address non-fatal errors using the Quality Improvement and Evaluation System (QIES)
Assessment Submission and Processing (ASAP) System MDS 3.0 Provider User's Guide.
Reference RAI Manual Chapter 5, Section 5.5, pages 5-7 through 5-14.
13. Will maintain validation reports in a transmittal notebook to be organized by month.
14. Will ensure the BO (Business Office) designee receives the initial AND final validation report
that acknowledges the acceptance of the MDS into the QIES ASAP (Assessment Submission and
Processing System).
15. Will facilitate receiving Validation Reports timely, the RAI Coordinator will transmit as
frequently (daily) as necessary to obtain timely validation of MDS acceptance into the QIES
ASAP data base.

16. Will facilitate the completion and updating of resident care plans within the MDS Department
and the IDT to reflect the resident’s most current needs.
17. Will identify residents in need of a Significant Change in Status Assessment based on criteria in
the RAI Manual Chapter 2, Section 2.6, pages 2-22 through 2-24.
18. Will coordinate the interdisciplinary care plan team meeting weekly and as needed.
19. Has knowledge of residents overall care needs, and knowledge of where residents reside in the
facility.
20. Will participate in facility staff education as it relates to the RAI process, and the coordination of
obtaining accurate documentation from direct care staff.
21. Will notify the facility Director of Nursing (DON) and/or Administrator of risk areas when coded
on the MDS; for example, D0200.1.I. is coded as 1-Yes or D05001.I. is coded as 1-Yes.
22. Will participate in quality improvement activities in the facility.
23. Will compile data reports based on MDS data as requested by the Administrator or DON.
24. Will update information for the Certified Nursing Assisstant (CNA) care plan/kardex.
25. Will utilize the facility’s software program and computer system to comply with federal
requirements for completion and encoding.
