Orientation

Head and neck

1. Inspect size, symmentry, shape, breathing lesions and facial expression


2. Palpate for bleeding, trender, swelling and mass on the head
3. Palpate occipital, post oricular, pre oricular, parotid submandibular, submental and
posterior limp nodes
4. Inspect for parotid artery for visible pulsations
5. Palpate carotid arteries one at a time
6. Auscultae carotid artery for humps and bruids

Eyes ears nose and sinus

1. Inspect eys for redness, sweeolling of vulvar conjunctiva and sclera


2. Chek papillary reaction and light accommodation
3. Test for extra ocular movements
4. Check for patency of airlow of nostrils and bleeding
5. Palpate for sinuses
6. Test clients hearing acuity right and left
7. Inspect gums and tounge for color consistency and lesions
8. Assest tounge for movement

Posterior thorax

1. Assess shoulder strength


2. Percuss lung field for resonance
3. Auscultate for breath sounds
4. Note quality of respirations and check rr
5. Percuss tendernesss over the posterior flank or vostoovertibeal angle
6. Blunt percussion

Anterior thorax

1. Inspect breast for symmetry, and retraction


2. Auscultate heart sounds aortic, pulpmonic etc
3. Obtain pulse rate
Qbdomen

1. Inspect for symmetry and contour


2. Quscultate bowel sounds
3. Start at right lower quadrant clockwise
4. Percuss for tympanic and dull sounds

Upper and lower extremity

1. Test for muscle strength of the upper extremity


a. Flexion with resistance
b. Extension with resistance
c. Cross over hand grip
2. Test muscle sgtrength of lower extremety
a. Knee flexion wid resistnce
b. Knee extension with resistance
3. Check for presence of edema sheen and ankle
4. Palpate dorsalis pelvis posterior tibyal pulses
5. Capp refill of the big toe
6. Check for dorsal flexion with resistance
7. Observer clients balance rombergs gtest
8. Check posture and gate
