QU ES LO QUE CONVIERTE A UN PACIENTE ASA 3 EN ASA 4? La presencia de una patologa cualquiera, que sea causa o resultado de una alteracin que compromete la integridad y la funcin de otros organos o sistemas, alteracin que ya se ha manifestado clnicamente y pone en riesgo la vida del paciente (enfermedad sistmica severa que es una amenaza constante para la vida).
Ejemplos ASA IV
Insuficiencia cardaca congestiva Angina pectoris inestable Disfuncin pulmonar, hepatica o renal avanzada.

Al hacer referencia al estado fsico segn la clasificacin de la ASA, se hace referencia al ESTADO FISICO REAL DEL PACIENTE, ACTUAL Y PRESENTE EN EL MOMENTO DE LA EVALUACION.
ASA Class 5

"The kind of operative procedure is not a part of the classification system because a physical status patient is still in that status if scheduled for an excision of a skin lesion with monitored anesthesia care or if scheduled for a pancreatectomy with general anesthesia. The operative risk is different because of the surgery, but the physical condition of the patient is the same preoperatively."

El compromiso del Mdico Anestesilogo es: 1.- Establecer el estado fsico real y presente del paciente. 2.- Si la situacin lo permite, deber hacer el Mdico todo lo necesario para que su paciente est en las mejores condiciones posibles de salud antes de someterlo al riesgo de la ciruga y la anestesia. 3.- Esto significa que las acciones del equipo mdico estarn dirigidas a que el ASA del paciente sea menor. 4.- Se traducir esto en una menor morbilidad y mortalidad perioperatoria, que es el objetivo primordial de la evaluacin fsica inicial.
