Cedar Lane Regional Park From Bel Air: take Rt.543 south and turn right onto Rt. 136 South. Turn right onto Cedar Lane. From I-95: Take Exit 80, Route 543 N. Turn left onto Rt. 136 South. Turn right onto Cedar Lane. Playing field is turf. Depending on registration turnout, format will be either round-robin games (2025 min) or one game with half-time. Mouth guard, shin guards, water, turf cleats & anything else you need for game play. Please wear a white shirt for the first night.

FIELD HOCKEY RULES


The Harford Summer Field Hockey League is a Harford County Parks and Recreation program is sponsored by the Churchville Recreation Council. As such, state and federal guidelines must be followed. Under federal guidelines, men are allowed to participate. All players must maintain play in a controlled and safe manner, under the jurisdiction of the game referee. The officials may enforce additional safety requirements as they deem necessary, due to field conditions, etc. 1. Mouth guards are mandatory. If you lose your mouth guard one may be purchased at the field. 2. Players are strongly encouraged to wear shin guards. 3. No jewelry may be worn during play. 4. Signing of the registration form signifies agreement of the enclosed Players Code of Conduct. 5. Rain delays/cancellations: Games will be played if at all possible. If thunder & lightning cause delays, games will pick up as soon as possible. Delays and/or cancellation of games after 4:00 PM are subject to the discretion of league coordinator, field referees and Cedar Lane Park. Cancellations due to inclement weather will be sent via email (sent to the address listed on this form) by approx.4:00 p.m. You may also check the Cedar Lane Regional Park web site for field status!

Harford County Department of Parks & Recreation - Churchville Recreation Council WAIVER AND RELEASE OF LIABILITY As a participant in the Churchville Recreation Councils Fall Field Hockey league, I, the undersigned, 1. Acknowledge and fully understand that each participant will be engaging in activities that involve risk or serious injury, including permanent disability or death, and severs social and economic losses which might result from their own actions, inaction or negligence, but the actions inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or reasonably foreseeable at this time. 2. Assume all the foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death. 3. Release, waive, discharge and covenant not to sue the Harford County Department of Parks and Recreation, Churchville Recreation Council, its respective administrators. Directors, agents, coaches, referees, other participants, volunteers and owners or leasers of the premises used for events, all of which are hereafter referred to as releasees from any and liability to each of the undersigned for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise. PLAYERS CODE OF CONDUCT The Churchville Parks and Recreation Council (Council) is committed to providing quality recreational activities in a safe and enjoyable environment. To fulfill this commitment, the Recreation Council has established the following Code of Conduct. The code requires that individuals, including, but not limited to spectators, coaches, registrants, and volunteers in the Council programs and activities, conduct themselves in reasonable manner. Individuals may lose their right to participate in program and activities for any of the following; 1. Failure to abide by program rules as established by the Council. 2. Failure to comply with the direction given by Council representatives in the performance of their duties. 3. Misuse, destruction, damage, or theft of Council property or the property of others. 4. Indecent or obscene conduct, including profanity and unsportsman-like conduct. 5. Any action which, in the judgment of the Council places oneself or others at risk. This includes, but is not limited to the following: verbal or physical sexual advances, including pressure for sexual activity; unwelcome sexually motivated touching, pinching patting or intentional brushing against; verbal harassment or abuse; and remarks or gestures of a sexual nature. 6. Possession, use or distribution of weapons, instruments used as weapons, fireworks or explosives. 7. Possession, use or distribution of alcohol (except by permit) or controlled dangerous substances. 8. Any action that disrupts or obstructs participation in a Recreation Council program or activity. 9. Any action that constitutes a violation of local, state or federal law. Violations of the code may result in disciplinary action including, but not limited to, the following: 1. Verbal warning 2. Limited suspension from programs and activities 3. Permanent expulsion from programs and activities An individual may face permanent expulsion without verbal warning if, in the judgment of the Council, the violation is significant enough to warrant such action.
