FORM INSTRUCTIONS AUDIT PLAN IA-002 Audit personnel charged with completing this form shall document all applicable information. Audit plan approval is required from the Chief Auditor and Director of Quality Assurance prior to contacting the auditee. Block : 1 2 3 Description:
Identify the audit plan number. Note: This will be the audit report number. Identify the date this audit plan was issued. Day Month Year (e.g., 6 July 2012 for July 6, 2012). The AUDIT PLAN has four sections; each section must be completed in order by the auditor or indicated personnel. Section 1 documented by the auditor when preparing audit plan. Section 2 documented by the auditor after consideration of the audit objective(s), scope and criteria. Section 3 signed to indicate the approval of the Director of Quality Assurance and the Chief Auditor to ensure the availability of management support. Section 4 signed to acknowledge the receipt of this document by the process owner. Identify the process (Core Process), area (e.g., facilities, physical location(s), buildings), department, or vendor undergoing the audit. Identify the objective (purpose) for the audit (e.g., compliance, improvement, corrective action follow-up) Identify the audit scope The audit criteria selected to accomplish an effective audit shall be identified. Identify the standards used for auditing and current version/revision level. Identify the most current revision of the quality manual. Identify any procedures and/or instructions and current revision levels. Prospective audit dates and times must be noted. Identify the selected start and completion dates for audit activity. List the times selected to conduct the audit activities. An audit team shall be selected, based upon the need to complete audit activities and the availability of trained personnel. Identify the person selected as team leader. Note: For single auditor audits, the auditor is the audit team leader. Identify any audit personnel assisting Identify any observers, guides or translators needed to accomplish the audit.
