HOSPITAL- ACQUIRED PNEUMONIA Nursing diagnosis: Impaired Gas Exchange r/t excessive secretions in the respiratory tract Nursing

Interventions: 1. Perform deep breathing exercises. 2. Perform coughing exercises 3. Postural drainage. Place patient in a position in favor of draining spec ific lobe of the lungs with excessive secretions. 4. Use of incentive spirometry to increase the lungs ability to hold in oxy gen promoting respiration. 5. Oxygen therapy as prescribed by physician. 6. Give antimicrobials as prescribed by the physician. Nursing diagnosis: Ineffective Airway Clearance Nursing Interventions: 1. Perform deep breathing exercise. 2. Perform coughing exercise to help clear out excessive secretions. 3. Promote increase in fluid intake to thin secretions and make them easier to r emove. 4. Clear out oral, nasal & tracheal airways through suctioning to facilitate bre athing. 5. Administer oxygen as prescribed by physician. Nursing diagnosis: Acute Pain Nursing Interventions: 1. Perform deep breathing exercises. 2. Splint when coughing. 3. Perform non-pharmacologic interventions such as music therapy. 4. Encourage ambulation to divert attention from pain. 5. Encourage to increase fluid intake. 2. Encourage to eat nutritious foods rich in vitamins and minerals. 3. Offer frequent small feeding to reduce the feeling of a distended stomac h. 4. Maintain good oral hygiene. 5. Promote pleasant, relaxed atmosphere for eating. Nursing diagnosis: Deficient Knowledge about the Treatment Regimen and Preventive Health Measures Nursing interventions:

1. Explain about the condition. 2. Give information about the medications to be taken. 3. Encourage patient to cooperate in doing activities such as relaxation te chniques and chest physiotherapy. 4. Teach as well the significant others on the care of the patient. 5. Explain to the patient things to expect in recuperating stages. Perform bronchial hygiene. 4. Give steroids as prescribed by the physician. 5. Give NSAIDS as prescribed by the physician to reduce inflammatory respon se. Nursing diagnosis: Hyperthermia r/t dehydration Nursing interventions: 1. Encourage to increase fluid intake. 2. Perform tepid sponge bath. 3. Encourage to eat nutritious foods rich in vitamin C and minerals. 4. Give antibiotics as prescribed by physician. 5. Give bronchodilators and oxygen supplementation as prescribed by the phy sician.
