As such, Maxim must obtain certain information about you or your company enduse in order to comply with various U.S. trade regulations. For example, the U.S. Export Administration Regulations (15 CFR Parts 730 to 774) may require an export license by the U.S. government for militaryendusesorendusers. In order for Maxim to make a proper license determination and complete required export documentation, it is essentialthatyourcompanyprovideasignedStatementofAssurance(pagetwoofthisletter)toMaximregarding theenduseandendusersoftheMaximproducts.Theenduserisdefinedastheultimateconsigneewhowillbe usingtheproduct(thisisnotadistributor).TheenduseisdefinedasthefinalproductthattheMaximproductwill be integrated into (e.g. automobile, radar system, kitchen appliances, etc.). Wecertifythatwewillnotdirectlyorindirectlyexport,reexport,transmit,orcausetobeexported,reexportedor transmitted,anyControlledItemstoanycountry,individual,corporation,organization,orentitytowhichsuchexport,re export,ortransmissionisrestrictedorprohibited. 2.
