If you had any traffic tickets, bring your DMV Driving Record (H-6 report) of 10 years. 2 photos (two identical passport-style color photographs, the photos must be 2 x 2). $680 check or money order, payable to Department of Homeland Security. (If 75 years or older, only $595) To apply for the USCIS fee waiver that removes the $680 USCIS cost, you must bring proof of income such as 2011 Income Tax Return, a letter from Social Security or Social Services to prove you receive benefits such as SSI, food stamps, or cash aid, or proof that you are getting unemployment benefits. Information about your current marriage and previous marriages: current spouses full legal name, current address, date of birth, date of marriage, Social Security number, A# (if any). If your current spouse is a US citizen, bring copy of citizenship certificate. If you or your current spouse were previously married, bring date of marriage, date marriage ended, and how marriage ended (copy of divorce/death/annulment papers). List of addresses, employment and/or schools for the past five years with start and end dates. (See back of this flyer.) List of all your trips out of the United States including departure and return dates. (See back of this flyer.) List of all your children in the WORLD: their full name, dates of birth, residency cards A#,and addresses. (See back of this flyer.) English/Spanish Documentation of all arrests and/or convictions, including FREE Pre-Screening reason for the arrest, date, place, and outcome/disposition. Bring ALL arrest and court documents, even if the charges were dropped.
