Goals: After series nursing interventions, the family will perform appropriate activities regarding the eradication of the breading sites of insects, rodents and other vectors. Objectives: After nursing interventions, the family will: a. Discuss the importance of eliminating the breeding site of vectors b. Know and determine the consequences of most common disease they may acquire c. Enumerate at least 3 treatments concerning the breeding sites of vectors

Objectives: After nursing intervention the family will: a. be aware and be more knowledgeable about the importance of proper sanitation especially at home; b. be able to eliminate the presence of these breeding sites of vectors c. recognize the causes of breeding sites such as to prevent the occurrence of diseases.

1. Discuss the importance and purposes of proper sanitation 2. Cite the causes and effects of the prevalence of these unwanted pests around the home. 3. Suggest alternatives/methods that would eliminate the breeding sites of vectors 4. Explore with the family the ways of improving home sanitation considering its limited resources: a. emphasize to the family the proper storage of food that may attract vectors b. instruct all family members to prevent accumulation of stagnant water around their home since this is a good breeding place for insects.

Goals: After the nursing intervention the family will be able to eliminate the cough and colds and will prevent the recurrence of the disease in the future. Objectives: After nursing intervention the family will: a. acquire adequate information about the disease, including signs and symptoms of the disease, immediate health care assistance and preventive measures; b. be aware on how to reduce the chances of spreading communicable diseases to other family members; c. utilize community resources openly available in resolving the condition experienced.

1. Discuss with the family the causes, effects and complications of cough and cold. 2. Provide adequate knowledge on the various ways of maintaining cleanliness in their surroundings. 3. Explain the importance of proper food preparation, good nutrition, rest and sleep in strengthening ones resistance against illness, so as to prevent occurrence of cough and colds 4. Cite ways in eliminating the disease and limiting the occurrence of transmission by suggesting courses of action such as medications (e.g. measures like the application of alternative medicines like lagundi if resources in the community is inadequate) and preventive measures such as covering the mouth when sneezing or coughing and proper disposal of nasal or oral discharges. 5. Promote proper personal and environmental hygiene among all members of the family. 6. Provide information on health centers in the vicinity for immediate care assistance.

FAMILY PROBLEM
Improper hygiene Subjective data: Dahil sa dami ng anak ko, minsan ang dudungis na nila. Mabuti na lang nandyan ang panganay ko na si Nene, siya yung nagbabantay sa dalawang kapatid niya. Objective data: Nene, her nine-year-old daughter cuddles her younger brother Jose who has flu at this time. She manages to feed her other sibling with bare hands without hand washing. The fingernails and toenails of these children were not trimmed properly and filled with dirt. The other two siblings came into the house sweating and their feet were smudged with mud. Jose suddenly wet his shorts and Nene must clean him up. The place wherein he peed was not cleaned but left only. The pillow that was affected by the urine was just placed outside for the sun to dry.

GOALS & OBJECTIVES

Goal of Care: Within 3 hours of nursing interventions, the family will be able to recognize the current home environment and health practices. They must be able to identify healthy practices and be able to practice them habitually. These hygienic measures are as follows: proper hand washing, proper waste disposal and proper house cleaning. Objectives: Within 3 hours of nursing interventions, the family will be able to: 1. Recognize the need for proper hand washing before and after meals as well as after using the toilet 2. Enumerate factors that promote in unhygienic practices 3. Be knowledgeable in ways on how to maintain hygiene 4. Accept the importance of proper hygiene in the activities of daily living 5. Exhibit the desire to change the current unhygienic practices

INTERVENTIONS
1. Check if the family is aware of their health practices. 2. Demonstrate the proper hand washing.

RATIONALE
> This will help the nurse to know the severity of the health problem. > The nurse must perform the proper hand washing technique so that the family will be able to see the proper technique. A return demonstration will be necessary so that the nurse can assess if the family members can absorb the lesson. > This step will enable the family members to know the consequences if health practices were not observed in their family. > This will be a way of keeping in touch with the family and facilitate them to be able to find concrete ways to achieve the goal of observing hygienic practices.

Objectives: After two home visits, the family will be able to: 1 . I d e n t i f y t h e different ways on proper disposal of garbage such as: a. Use of compost pit with cover; b. Segregate the non-biodegradable and biodegradable materials; c. Recycling of can-be-used garbage; d. Reusing or selling of some garbage like cans, bottles and plastics. 2 . E n u m e r a t e the proper techniques on keeping the surroundings clean and through using proper method of waste disposal; 3.Define the meaning of proper garbage disposal and its advantages; 4. Recognize the possible effects of garbage burning; verbalize understanding about the importance of practicing proper waste disposal.
