1. PURPOSE :
1.1 To assist patient for discharge that their needs are met and continuity of care is not inter rupted.

2. DEFINITION : 2.1 Discharge Planning is the process by which the patient is assisted to develop a plan of care
for ongoing maintenance and improvement of health care even after he/she maybe dis charged from the acute care setting.

POLICY :

3.1 All patients will be assessed at the time of administration to determine their discharged needs. 3.2 The discharged planning process will be integrated and coordinated by health care professio nals and shall include the patient, family and significant others.

4.

PROCEDURES:

4.1 All patients will be screened for discharge planning needs during the admission process. 4.2 The admitting nurse will identify potential discharge planning needs consistent with the Physicians plan of care. 4.3 Assessment data will be obtained from the patient or significant others and include considera tion to nutritional, functional, physical, psychosocial, environmental, spiritual, self care, safety, pain, education and other factors that will enable the interdisciplinary team, to develop the most appropriate discharge plan for the patient. 4.4 Factors included in the initial assessment for discharge planning are : 4.4.1 Medical condition. 4.4.2 Continuing care needs. 4.4.3 Assistance required with activities of daily living. 4.4.4 Living arrangements prior to admission.
