BIENESTAR

JOB TITLE: EXEMPT/NON EXEMPT: POSITION DESCRIPTION: CRCS Counselor Non Exempt Under the direction of the Program Director the CRCS counselor will focus on carrying out program activities to meet contractual agreements set by Bienestar on behalf and for clients. The CRCS Counselor assesses the prevention needs of Transgender clients at high risk, HIV positive or living with AIDS. Implements and monitors an agreed upon service plan for each client and assists clients in accessing needed services.

DUTIES/RESPONSIBILITIES: 1. Conducts a comprehensive initial assessment on each client, which includes obtaining prevention needs, physical, psychosocial, environmental, eligibility and financial information and evaluating the clients strengths, needs, and available resources. Develops, in conjunction with the client, an individual prevention service plan based on the information from the initial assessment; monitors the implementation and progress of this service plan. Updates the individual service plan as frequently as needed by changes in the clients status, but no less than every six months. Conducts case conference reviews in a manner that ensures that each active client is discussed a minimum of once every six months. Acts as a liaison between Bienestar and other community based service providers; advocates on behalf of the client in overcoming obstacles to service being provided to client. Assists and refers clients to services, including but not limited to food, shelter, transportation, medical care, benefits, counseling, and emotional support; for more difficult cases, refer clients to agency and/or community based specialists for assistance. Maintains face-to-face and telephone contact with clients based on the clients acuity level; reassesses clients strengths, needs, and available resources with every client contact.

Maintains complete, accurate, and timely client case records; progress notes are signed and dated; releases and consents are updated as needed. Provides on call department coverage, as directed. Participates in staff and community meetings Other duties as assigned.

Sensitivity to the needs of diverse cultures and populations, the issues of HIV high risk negative as well as HIV positive persons or living with AIDS Current knowledge of HIV services. Experience working with culturally and socially diverse community groups including impacted and affected HIV individuals and groups; and the underserved. Bachelors degree or comparable professed knowledge in psychology, sociology, social work or related field preferred. Bilingual (Spanish) and/or bicultural required. Ability and willingness to work in a team oriented work environment, with professionals and paraprofessionals from different disciplines, as well as volunteers and clients. Able to meet agency job work hours and transportation requirements.
