AUDIO TECHNOLOGY DIPLOMA PROGRAM professional audio, as well as the media arts, and must be a high school graduate or GED recipient. Applicant must be at least 18 years of age. Applicants are required to pass a multiple-choice test evaluating general English and Math To be admitted to SAE Institute and its programs, an applicant must complete the following process: 1. COMPLETE THE APPLICATION FOR ADMISSION Complete the application and enrollment agreement in full. Return completed application and enrollment agreement to SAE Atlanta with the non-refundable $50 application fee. Provide proof of High School graduation or Transcript / GED or bring the original High School Diploma to the SAE campus of enrollment. COMPASS scores directly to SAE Atlanta. The above listed school and student enter into an agreement under which the school will instruct the student in the curriculum listed below in accordance with the Education Law and Commissioners Regulations. The student will pay tuition and fees as listed below and will adhere to the schools rules and regulations as set forth in the school Catalog.

ADMISSION
Students at SAE Institute of Technology are admitted without regard to race, color, creed, sex, age or national origin. The school reserves the right to accept or reject all applicants. In the event that an applicant does not meet admissions requirements, or is rejected for training, the applicant will be noti ed in writing, and all previous obligations within this contract will be void. All monies paid by the applicant will be returned within 30 days and there will be no future liability.

AUDIO TECHNOLOGY ASSOCIATES OF SCIENCE DEGREE PROGRAM professional audio and the media arts, and must be a high school graduate or GED recipient. Applicant must be at least 18 years of age. Applicants are required to pass a multiplemust also be submitted to SAE Atlanta. Students must have completed 15 Semester Credit Hours of General Education. These credits must be completed with a C (2.0 on 4.0 scale) average at an approved accredited institution prior to enrolling at SAE Atlanta. To meet institutional approval, the institution must be accredited by agencies recognized by either the Department of Education or institutions that operate under state approval. At least one course is required in each of the following: 1. Mathematics such as algebra, trigonometry, a n a l y t i c geometry, calculus, statistics and other higherlevel mathematics. 2. Science such as biology, botany, chemistry, ecology and physics. 3. Humanities such as English, literature, philosophy, logic, history of art and sciences, communications, composition, speech and drama and combinations of courses. 4. Social Science such as history, psychology, government and sociology. *Please note in certain situations it is possible to commence enrollment in the Diploma Program and transfer to the Associate of Science Degree Program. If any installment is not paid when due, all remaining installments shall, at the option of the holder, become immediately due and payable. Each payment is due within the rst 7 days of the month. A $5.00 per business day Late Fee will be assessed for any payment made after the seventh day of the month. SAE o ers a credit of 3% on all program tuitions paid in full in advance of the rst class, with the exception of any loans administered through SAE (i.e., SLM) or any third-party agency sponsorship (i.e., VESID). Should the student choose to nance the tuition through Sallie Maes Career Training Loan Program, please see the separate Disclosure Statement and Promissory Note from SLM as part of this contract.

CANCELLATION AND REFUND POLICY


All refund policies re ect compliance with speci c state law and post-secondary authorization and/or licensing rules for students enrolled on SAE campuses. All refund policies will be printed in the SAE Catalog and on this Enrollment Agreement and re ect state requirements for the campus of enrollment. In all situations, it is SAEs policy to return all monies paid including the Application Fee, if the student is denied enrollment by the school, or if any scheduled class start is cancelled for reasons of insu cient enrollment. A student can be dismissed, at the discretion of the Director, for insu cient progress, nonpayment of costs and/or failure to comply with SAE rules and regulations. A students may cancel his/her enrollment in writing at any time, in which case the following conditions will apply: A. All monies paid by an applicant must be refunded if requested within three (3) days after signing an enrollment agreement and making an initial payment, or if no contract was signed and prior to classes beginning, the student requests a refund within three (3) business days after making a payment. B. A student who cancels more than three (3) days after signing the enrollment agreement, but before instruction begins, receives all monies returned with the exception of the application fee. C. Thereafter, a student will be liable for: 1. the application fee, plus 2. the cost of any textbooks or supplies accepted by the student and are considered non-refundable, plus 3. tuition liability as of the students last date of physical attendance. If student termination/withdrawal occurs during the first week of the program, the student owes no tuition liability. Thereafter, a pro-rata refund policy will be used in which the student will only be charged for those hours actually o ered to the student. Hours o ered refers to the number of hours o ered between the rst day of class and the students last date of physical attendance.
Notice of cancellation or withdrawal is requested in writing. Refunds will be made within 30 days after receipt of notice of cancellation. All refunds will be issued to the enrolled student regardless of how tuition was paid (with the exception of any loan or agency payments; in these cases, the refund will be issued to loan company or agency). Any tuition balances remaining on account after withdrawal or termination must be paid in a timely manner or will be subject to collection. Any payment arrangement made to pay such balances may be subject to additional fees and/or interest. REFUND ON TEXTBOOKS AND/OR MATERIALS: Special order materials such as the computer, software, headphones and audio interface are considered non-refundable materials and will only be refunded if not accepted by the student or are returned to SAE in the unopened facotry sealed packaging. Students will receive a full refund on any textbooks and/or materials not accepted by the student. Textbooks are considered refundable only if they are returned to SAE in a condition which does not prevent them from being used by or sold to new students.

ACKNOWLEDGEMENTS
By my signature, I agree to the conditions of this agreement. I certify that this contract contains all the terms of our agreement and that there have been no promises or agreements made other than those contained herein. I also verify that I have read and received a copy of this Agreement and the school Catalog. I also understand that, although career development/placement assistance service is provided, the school cannot guarantee a job to any student or graduate. This agreement shall be e ective when signed by the schools representative.

Any holder of this consumer credit contract is subject to all claims and defenses which the debtor could assert against the seller of goods or services obtained pursuant hereto or with the proceeds thereof. Recovery hereunder by the debtor shall be limited to the amounts paid by the debtor hereunder. You, the buyer, may cancel this transaction at any time prior to midnight of the third calendar day after the e ective date of this transaction, provided you have not yet begun training. SAE regularly upgrades its equipment, software, materials and instructional content in order to keep up with changing technology. SAE reserves the right to upgrade its facilities, equipment, curriculum and other student materials at any time to provide the student with industry-standard practical experience. Degrees, Diplomas, Certi cates and Transcripts will not be issued by SAE until all graduation requirements and monetary obligations have been met. page 2 of 2 pages.
