Hyoscine butylbromide

Generic Name: Hyoscine butylbromide (scopolamine butylbromide, butylhyoscine, butylscopolamine) Brand Name: Buscopan, Ancopan, Antipen, Bromid, Brospan, Buscogen, Buscon, Colipan, Hylac, Hysocin, Lopan, Spanil, Typan Available Forms: Capsules: 0.25mg Tablets: 10mg Suppositories: 10mg Injections Transdermal Patch (Scopolamine) Indications: * Spastic States * Delirium, Preanesthetic sedation, and obstetric amnesia with analgesics * To prevent nausea and vomiting from motion sickness * Sometimes used to reduce secretions in the lung Mechanism of Action: Inhibits muscarinic actions of acetylcholine on autonomic effectors innervated by postganglionic cholinergic neurons. May affect neural pathways originating in the inner ear to inhibit nausea and vomiting. Hyoscine butylbromide works by stopping spasms in our smooth muscle (walls of the stomach, intestines, bowel, bile duct and urinary tract) by preventing acetylcholine, a neurotransmitter causing the contractions on the muscle, to act on the muscles.

Hyoscine butylbromide is not an analgesic as it does not mask or cover the pain but rather it works to prevent painful cramps and spasms. Adverse Reactions: CNS: disorientation, restlessness, irritability, dizziness, drowsiness, headache, confusion, hallucinations, delirium, impaired memory. CV: palpitations, tachycardia, paradoxical bradycardia, flushing EENT: dilated pupils, blurred vision, photophobia, increased intraocular pressure, difficulty swallowing GI: constipation, dry mouth, nausea, vomiting, epigastric distress GU: urinary hesitancy, urine retention Respiratory: bronchial plugging, depressed respirations Skin: rash, dryness, contact dermatitis (with transdermal patch) Effects on Lab Test Results: * None reported. Contraindications and Cautions: * Contraindicated in patients with angle closure glaucoma, obstructive uropathy, obstructive disease of the GI Tract, asthma, chronic pulmonary disease, myasthenia gravis, paralytic ileus, intestinal atony, unstable CV status in acute hemorrhage, tachycardia from cardiac insufficiency or toxic megacolon * Use with caution in patients with autonomic neuropathy, hyperthyroidism, coronary artery disease, arrhythmias, heart failure, hypertension, hiatal hernia with reflux esophagitis, hepatic or renal disease, known or suspected GI infection, or ulcerative colitis * Use with caution with children younger than 6 years old

* Use with caution with patients in hot or humid environment (drug may cause heat-stroke) Responsibilities: * Raise side rails as precaution because some patients become temporarily excited or disoriented and some develop amnesia or become drowsy. Reorient as needed. * Tolerance may develop when therapy is prolonged. * Atropine-like toxicity may cause dose-related adverse reactions. Individual tolerance varies greatly. * ALERT! Overdose may cause curarelike effects, sucj as respiratory paralysis. Keep emergency equipments available. * Avoid activities that require alertness until CNS effects of drug are known. * Tablets may be taken before, during, or after meals.
