I, the complainant in this case, state that the following is true to the best of my knowledge and belief.

From in or about August 2012, to on or about April 24, 2013, in the county of Oklahoma, in the Western District ofOklahoma,
the defendants violated:

This criminal complaint is based on these facts:

AFFIDAVIT OF TASK FORCE OFFICER JERE J. HOLT
DRUG ENFORCEMENT ADMINISTRATION
1. I, Drug Enforcement Administration Task Force Officer (DEA TFO) Jere 1. Holt,
being duly sworn, do hereby state that I am an Investigator with the District 21 District
Attorney's Office, having been so employed since July, 2008. During my time with the District
Attorney's Task Force I have been assigned to the Investigations Division. I am currently on a
special assignment as a Task Force Officer with the Drug Enforcement Administration (DEA)
and have been so since July, 2008. Prior to my employment with the District 21 District
Attorney's Office, I was employed as a Senior Probation and Parole Officer with the Oklahoma
Department of Corrections from September 2005 through June 2008. During my tenure as a
probation and parole officer, I attended the Oklahoma Council on Law Enforcement Education
and Training Basic Peace Officer Academy, also referred to as C.L.E.E.T., and subsequently
earned my license as a certified peace officer for the State of Oklahoma.
2. During my employment in law enforcement, I have received training, and conducted
investigations, related to illicit drugs, and the distribution of illicit drugs, in violation of Title 21,
United States Code, Sections 841(a)(1), 843(b), 846, and others. I have participated in many
aspects of drug investigations including the following: undercover operations; surveillance;
search warrants; arrests; reviewing taped conversations and drug records; and debriefing
defendants, informants, and witnesses who had personal knowledge regarding major narcotics
trafficking organizations.
3. This affidavit is submitted in support of a criminal complaint charging Anna GAL VAN,
Luis LEON-SANCHEZ, Jesus GUTIERREZ-ZAZUETA, Omar COTA-SANCHEZ, and
Jesus LEON-SANCHEZ with conspiracy to possess with intent to distribute 1000 grams or

more of a mixture or substance containing a detectable amount of heroin in violation of Title 21,
United States Code, Section 846. My knowledge of infonnation contained in this affidavit is
from my direct participation and discussions with other agents directly involved in this
investigation. The facts contained in this affidavit are offered to establish probable cause, and do
not represent every fact known to myself or other participating agents.
4. In August of 2012, agents with the Drug Enforcement Administration Oklahoma City
District Office (OCDO) initiated an investigation into a heroin drug trafficking organization
operating in South Oklahoma City, Oklahoma. During the course of this investigation, agents
identified Anna GALVAN as a leader within the organization who was directly responsible for
the distribution, facilitation, and coordination of heroin in Oklahoma City, Oklahoma. Anna
GALVAN was previously identified in an earlier investigation in which Miseal GARCIA, the
father of her child, was arrested and subsequently convicted for conspiracy to distribute heroin
following the discovery of more than 600 grams of black tar heroin in the residence occupied by
Anna GALVAN and Miseal GARCIA.
5. On April 25

the Oklahoma City Police Department Santa Fe IMPACT Unit executed a search warrant at 321
S.E. 25

Street, Oklahoma City, Oklahoma, 740 Santa Rosa Drive, Oklahoma City, Oklahoma,
and 1202 S.W. 74

Street Apartment 262, Oklahoma City, Oklahoma. These residences were
identified as residences associated with the Anna GALVAN heroin drug trafficking organization
(DTO) operating in Oklahoma City, Oklahoma. On April 24

Troung, District Judge for Oklahoma County. Included in the items to be seized in the search
warrant were heroin, scales, monies derived from the sale of narcotics, ledgers and other types of
documentation regarding the sale of narcotics, and items used to distribute the sales of narcotics
to include baggies.
6. At approximately 6:30 a.m., agents executed search warrants at 321 S.E. 25

Oklahoma City, Oklahoma, 740 Santa Rosa Drive, Oklahoma City, Oklahoma, and 1202 S.W.
74th Street Apartment 262, Oklahoma City, Oklahoma. During the execution of these search
warrants agents detained Anna GAL VAN and Jesus GUTIERREZ-ZAZUETA at 321 S.E.

7. During the search of 321 S.E. 25
th
Street, Oklahoma City, Oklahoma agents discovered in
excess of 150 grams of a black tar substance suspected to be heroin, ledgers, and the cellular
telephone agents previously identified as the dispatch phone utilized to take orders from heroin
users. Agents also discovered a .22 caliber semi-automatic pistol and a Bushmaster AR-15 Semi
Automatic Rifle. The Bushmaster AR-15 semi-automatic rifle was discovered in the trunk of a
green Dodge Intrepid with the suspected heroin. During the search of 1202 S. W. 74

Apartment 262, Oklahoma City, Oklahoma, agents discovered 182 multi-colored balloons
containing a black tar like substance of suspected heroin, a ballistic vest, a digital scale, and
multiple empty multi-colored balloons. During the search of 740 Santa Rosa Drive, Oklahoma
City, Oklahoma, agents discovered handwritten figures consistent with a drug ledger, a cellular
telephone previously identified as a number used to distribute heroin, a Springfield .45 caliber

semi-automatic handgun, and numerous empty balloons similar to the balloons used to package
heroin.
8. During a post Miranda interview with Anna GALVAN, GALVAN stated she obtains
12-20 pieces of heroin approximately two times per month from Las Vegas, Nevada and has
done so for the past eight to nine months. GALV AN defined a "piece" as weighing
approximately 25 grams. GALVAN stated she and Jesus GUTIERREZ-ZAZUETA were
recently married approximately one week ago in Oklahoma County. GALVAN identified a
photograph of Jesus LEON-SANCHEZ as "Jose" and stated he was responsible for storing
heroin and had recently stored approximately seven "pieces" of heroin at her residence located at

321 S.E. Street, Oklahoma City, Oklahoma at her request. GALVAN identified a
photograph of Omar CATO-SANCHEZ as "Omar" and stated he was responsible for
delivering heroin to heroin users after the users contacted the dispatch number. GALVAN
identified a photograph of Luis LEON-SANCHEZ as "Jose" and stated he was responsible for
answering the dispatch phone heroin customers placing their orders and would advise Omar
CATO-SANCHEZ about where to meet the heroin customer. GALV AN stated she paid Luis
LEON-SANCHEZ $150.00 per day for answering the dispatch phone, Omar CATO
SANCHEZ $150.00 per day for delivering heroin to heroin customers, and paid Jesus LEON
SANCHEZ $100 per day for overseeing the storage of the heroin. GALVAN stated she paid the

Oklahoma. When the Springfield .45 semi-automatic pistol was discovered, Jesus
GUTIERREZ-ZAZUETA made an utterance and stated the firearm belonged to him.

9. During a post Miranda interview with Luis LEON-SANCHEZ, LEON-SANCHEZ
stated he was recruited to be a heroin dispatcher by "Jube" a.k.a. Jesus GUTIERREZ
ZAZUETA, in January of 2013. Luis LEON-SANCHEZ stated he was taught how to answer
the phone and fill heroin customer's orders by GALVAN. Luis LEON-SANCHEZ stated
GALVAN and Jesus GUTIERREZ-ZAZUETA instructed him on the price of a small balloon
of heroin as $40.00 and a big balloon of heroin as $80.00. From your affiants training,
experience and knowledge of this investigation, a "small" is coded language used to describe .2
grams of heroin and a "big" is coded language used to describe .4 grams of heroin. Luis LEON
SANCHEZ stated GALVAN paid him $100.00 per day for answering the dispatch phone and
paid his rent and bills at 321 S.E. 25

Street, Oklahoma City, Oklahoma.
10. During a post Miranda interview with Omar CATO-SANCHEZ, CATO-SANCHEZ
stated he was a delivery driver for Anna GALVAN and was recruited by Jesus LEON
SANCHEZ. CATO-SANCHEZ stated he delivered heroin approximately 8-10 times per day
for the last ten days. CATO-SANCHEZ stated he stayed at 1202 S.W. 74th Street, Oklahoma
City, Oklahoma with Jesus LEON-SANCHEZ which is where he would take the proceeds from
the heroin sales to following his deliveries. CATO-SANCHEZ stated he was paid $80.00 per
day at Anna GALVAN's direction.
11. During a post Miranda interview with Jesus LEON-SANCHEZ, Jesus LEON
SANCHEZ stated he was recruited to deliver heroin by Jesus GUTIERREZ-ZAZUETA who
Jesus LEON-SANCHEZ knows as Anna GAL V AN's husband. Jesus LEON-SANCHEZ
stated he was given a vehicle by Jesus GUTIERREZ-ZAZUETA approximately three weeks
ago and was paid $100.00 per day to safeguard GALVAN's heroin. Jesus LEON-SANCHEZ
stated approximately one week ago he was given a bag containing heroin by GALVAN and

instructed to hide the heroin. Jesus LEON-SANCHEZ stated he stored the heroin in the trunk of
a green car behind the residence of321 S.E. 25

12. In summary, based upon my training and experience as a law enforcement officer, and
the above mentioned facts, I submit there is probable cause to believe that Anna GALVAN, Luis
LEON-SANCHEZ, Jesus GUTIERREZ-ZAZUETA, Omar COTA-SANCHEZ, and Jesus LEON
SANCHEZ conspired to possess with intent to distribute 100 grams or more of a mixture or
substance containing a detectable amount of heroin, a Schedule I controlled substance, in
violation of Title 21, United States Code, Section 846.
