EXTENSION SERVICE REPORT NACTIVITY: DURATION: Blood Letting (Date of project implementation)

PROPONENTS: Red Cross Youth RECIPIENTS: Those who are in need of blood and the Blood Bank

PROJECT OUTPUT DESCRIPTION The Blood letting was held at the Mapua Institute of Technology Makati last August, 2012. The Activity. The Blood Letting Activity is partnered with the volunteers from the Philippine National Red Cross Rizal Chapter. Students and Employees of the school are welcome to donate their blood. The donors were assisted by the nurse and screened by the doctor from The Philippine National Red Cross Rizal Chapter. The nurse assisted the qualified donors and the blood donation was conducted. The Activity last for a day from 8 am to 5 pm. The beneficiaries of the activity are those people who are being helped by the Philippine Red Cross some blood are carried over to the blood bank for later use if anyone is in need. The activity was a success since many students and employees participated in the activity. The students and employees even the people who have other business that is inside the school and seen the activity were encouraged to be screened if qualified and donate their blood. Details: Report should be in narrative form and must contain the what, where, when and how of the project implementation. The purpose of this is to clearly identify what transpired during the conduct of the activity. Include quality pictures at the end of the narrative report. Each picture should be accompanied with a descriptive caption. Minimum of eight (8) pictures should be attached in the report, two (2) pictures per page with a size of 4 x 5 for each picture.
