CAUSES.Asthmacanbeclassifiedintothreetypesaccordingtocausativefactors. Allergic oratopicasthma(sometimescalledextrinsicasthma)isduetoanALLERGYto ANTIGENSusuallytheoffendingallergensaresuspendedintheairintheformof pollen,dust,smoke,automobileexhaust,oranimaldander.Morethanhalfofthe casesofasthmainchildrenandyoungadultsareofthistype.Intrinsicasthmais usuallysecondarytochronicorrecurrentinfectionsofthebronchi,sinuses,or tonsilsandadenoids.Thereisevidencethatthistypedevelopsfroma HYPERSENSITIVITYtothebacteriaor,morecommonly,virusescausingtheinfection. Attackscanbeprecipitatedbyinfections,emotionalfactors,andexposureto nonspecificirritants.Thethirdtypeofasthma,mixed,isduetoacombinationof extrinsicandintrinsicfactors. SYMPTOMS.Typically,anattackofasthmaischaracterizedbydyspneaanda wheezingtypeofrespiration.Thepatientusuallyassumesaclassicsittingposition, leaningforwardsoastousealltheaccessorymusclesofrespiration.Theskinis usuallypaleandmoistwithperspiration,butinasevereattacktheremaybe cyanosisofthelipsandnailbeds.Intheearlystagesoftheattackcoughingmaybe drybutastheattackprogressesthecoughbecomesmoreproductiveofathick, tenacious,mucoidsputum. TREATMENT .The treatmentof extrinsicasthma beginswith attemptsto determinethe allergenscausing theattacks.The cooperationofthe patientisneededto relateonsetof attackswithspecific environmental substancesand emotionalfactors thattriggeror intensifysymptoms. Thepatientwith nonallergicasthma shouldavoid infections, nonspecificirritants, suchascigarette smoke,andother factorsthatprovoke attacks. Drugsgivenforthe treatmentofasthma areprimarilyused forthereliefof symptoms.Thereis nocureforasthma butthediseasecan becontrolledwithan individualized regimenofdrug therapycoupledwith rest,relaxation,and avoidanceof causativefactors. Anasthmaattackwithrespiratorydistress.FromFrazieretal., 2000. Bronchodilators suchasepinephrine andaminophyllinemaybeusedtoenlargethebronchioles,thusrelievingrespiratory embarrassment.Otherdrugsthatthinthesecretionsandhelpintheirejection (expectorants)mayalsobeprescribed. Thepatientwithstatusasthmaticusisveryseriouslyillandmustreceivespecial attentionandmedicationtoavoidexcessivestrainontheheartandsevere respiratorydifficultiesthatcanbefatal. PATIENT CARE.Becauseasthmaisachronicconditionwithanirregularpatternof remissionsandexacerbations,educationofthepatientisessentialtosuccessful treatment.Theplanofcaremustbehighlyindividualizedtomeettheneedsofthe patientandmustbedesignedtoencourageactiveparticipationintheprescribed programandinselfcare.Mostpatientswelcometheopportunitytolearnmoreabout theirdisorderandwaysinwhichtheycanexertsomecontrolovertheenvironmental andemotionaleventsthatarelikelytoprecipitateanattack. Exercisesthatimproveposturearehelpfulinmaintaininggoodairexchange.Special deepbreathingexercisescanbetaughttothepatientsothatelasticityandfull expansionoflungandbronchialtissuesaremaintained.(SeealsoLUNGandCHRONIC OBSTRUCTIVE PULMONARY DISEASE.)Someasthmaticpatientshavedevelopeda protectivebreathingpatternthatisshallowandineffectivebecauseofafearthat deepbreathingwillbringonanattackofcoughingandwheezing.Theywillneedhelp inbreakingthispatternandlearningtobreathedeeplyandfullyexpandthebronchi andlungs. Thepatientshouldbeencouragedtodrinklargequantitiesoffluidsunlessotherwise contraindicated.Theextrafluidsareneededtoreplacethoselostduringrespiratory distress.Theincreasedintakeoffluidsalsocanhelpthinthebronchialsecretionsso thattheyaremoreeasilyremovedbycoughinganddeepbreathing. Thepatientshouldbewarnedofthehazardsofextremesineating,exercise,and emotionaleventssuchasprolongedlaughingorcrying.Thekeywordsare modificationandmoderationtoavoidovertaxingandoverstimulatingthebody systems.Relaxationtechniquescanbeveryhelpful,especiallyifthepatientcan findamethodthateffectivelyreducestension.
