I. ACKNOWLEDGMENT First of all, I would like to thanks those people who support and help me to complete my PRACTICUM II. They help me to gain more knowledge about their doings inside the office, even though they have work they have time to teach me and to guide me at all times. Thats why Im thanking to them because I have more knowledge to prove that I have improvement to myself from them. To Mrs. Griar and to all staff of Central Records Sections, Id like to express my deep appreciation to them for a good relationship always even though I made a mistaken in work they always smile at me and guide me how to correct my wrong work. OBSERVATIONS IN THE COMPANY What I have observed at Central Records Section is they are so responsible in their work and very careful to the papers that they receive and to the information that they give to each divisions. Their files are always in the proper places and they are always in good mood in the clients. The strength and challenges of this company is that they are so good when it comes in socializing. They are talking to their patients well and in good manners, thats why they are so effective in the job. They are very helpful in their patients and employees. I think the best practice of this company is their managing process when it comes to the files and records of its company and also their attitude in their patients, co-workers and to the other people. My over-all observation in Lung Center is all employees are very happy because they are always smiling at me and because of that I feel good and it helps me to work effectively. Especially to the doctors and nurses, they do their work happily and they do their best to help their patients more than the patients need. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: Filing documents (contracts, application for leave & etc.) Updates emails. OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: To be professional like them and to be a good worker and to be good when I have clients. I also learned how to use touch screen duplicator, to duplicate the documents of the patients for a sense reasons & their shredder for the papers that are already used. B. Equipment, tools, testing apparatus handled (soft wares used included) Microsoft Excel, to update the emails of the patients and employees. Take care all the important things by updating it. C. Strong points versus weak points The strong points is they are always serving to the mass and their attitude was so nice for me like theyre always smiling at me. The weak points is their hallway because of the fire few years ago, it must be renovated and their facilities must be clean.

We were happy when we are doing our work. I learned how to manage my mood when it comes to clients, this is the one thing that I adopt from them. E.
