Amended Request to Add a Conservator

Pursuant to the Honorable Court's April 7, 2013 Decision, the Requester herein files Amended Request to Add a Conservator. Following are the reasons: 1. Pursuant to the November 23, 2011 Decision by Presiding Judge Ehud Rekem, order was issued, appointing Mr Robert Zernik Conservator for the body and property of Ms Rivka Shami Zernik (The Conservatee). Attached is a copy of the November 23, 2011 Order, marked Appendix A. 2. It should be clarified that the Conservatee's husband, Conservator Robert Zernik, is the Conservatee's husband, who care for her with love and unending devotion. 3. With it, it should be noted that the Conservatee's husband, Conservator Robert Zernik, is not a young man, at about 85, but still full of vigor. 4. Under such circumstances, and since the Requester is not getting any younger, it is clear that it is beneficial to appoint an additional conservator, who would lighten the burden on the Conservator even a little bit. The existing Conservator will continue to care for the interests of his beloved wife also in the future. 5. Under such circumstances, it is clear that the Conservator and the Conservatee's son, Mr Dror Zernik, who today shares with his father the burden of care for the Convervatee, is the natural candidate to share the role of Conservator with his father, for the care of his beloved mother, unfortunately a Conservatee. 6. It should also be noted that already in 2008, when her medical condition started deteriorating, the Conservatee signed authorization for Mr Robert Zernik (her husband) and Mr Dror Zernik (her son), pertaining to her medical care. Under such circumstances, it is possible to learn of the Conservatee's will, based on the identities of those authorized, prior to the deterioration in her medical condition. 7. Both the Requester, the Conservator, and the Conservatee's son, Dror Zernik, agree to the adding of a Conservator, and will jointly work for the benefit of the Conservatee. Attached is Affidavit of Consent of Mr Dror Zernik to be appointed Additional Conservator for the body and property of the Conservatee, marked Appendix B.

8. Therefore, the Court is asked to order the appointment of Mr Dror Zernik 0652... , as an additional Conservator for the body and property of the Conservatee, together with his father, Mr Robert Zernik, Conservator for the body and property of the Conservatee. 9. It is just and correct to grant instant request. 10. Attached is the Requester's Affidavit verifying the facts underlying instant Request. 11. Attached is the son's Dror Zernik - Consent to be appointed joint additional Conservator.
