Planning Ahead: Medical Directives & Emergency contacts

Life Care Planning is an important task for all of us, whether young or old, healthy or facing challenges. Through increased awareness and access to information, Arizonans of all ages can make their choices known about who will manage their affairs in the event of an emergency. At Fellowship Square, Residents are encouraged to utilize a Pre-Hospital Medical Care Directive and post it on your refrigerator or on the inside of your front door. Sometimes this is called a DNRDo Not Resuscitate. The PreHospital Medical Care Directive is a document signed by you and your doctor that informs emergency medical technicians (EMTs) or hospital emergency personnel not to resuscitate you. According to Arizona law, the DNR form is printed on letter-sized, orange paper. Blank DNR forms are available from the Activities Office in Villa 1, or by contacting Resident Services: 721-3006. In addition, Fellowship Square require all Residents to have a current Emergency Contact form on file with our Leasing Office. Please take a few moments to review that your Emergency Contact information is correct and up-to-date. The Emergency Contact form has an area to update any medications that may have changed throughout the year. This information is extremely helpful if emergency medical personnel are called and you are unable to communicate. Please make yourself a copy of your emergency information and place it on your refrigerator; this information is vital in an emergency situation. Emergency Contact forms are available from the Leasing office in Villa 2, or by calling Residents Services at 721-3006. Additional life care planning forms and services are available free of charge through the AZ Attorney Generals website at: www.azag.gov/seniors/life-care-planning. For General Durable Power of Attorney or other financial documents, it is recommended that you contact a private attorney for assistance with these and other serious legal matters. Christian Care Foundation can also help with estate and financial planning. For more information contact Bill Tennison, Development Director, Christian Care Foundation: (480) 234-9536.

THE 4TH OF JULY By Ellen Canterbury
The Declaration of Independence brought before us a newness, a wonder, an unending joy. We live in the land of the free. No tyrant, no tyranny can overcome what we cherish. In the home of the brave, we rest not on our laurels, but daily we strive to make for others, a land of the free. Hard-fought battles have secured this freedom. Now may the glory spread throughout the world, that all may know the joy of freedom.

CINQUAIN FOR LAMENT By Dotty Dittmar

Boston, Denver, Newtown. Columbine in the past. Violence unbelievable we hurt.

COME WITH ME By Roger Kase

Come with me Dart and roll like sleek brown sea otters in the gurgling surf Cracking the abalone of life open on the stone of hopes and dreams. Each of us carries through the waves upon our chests.

YOU By Pat Zwerk

We all had a father, We all had a mother, And some us later Became one or the other. Whether reared just by one, Or lucky to have the two, The genes you inherited Helped make you you. They gave you your looks From your had to your toes, And they gave you the chance To live life as you chose. If you made some bad choices You made good choices too And the sum of it all Is whats made you you.

LIBRARY CORNER
A book is only half read until it is discussed. When I was a teacher of literature, this was the single most method of getting students to fully understand what they read. The ideas in a book, story or poem should be shared with others. If a book is worth reading, it should be worth discussing. This is why the classics are still favorites even after centuries. Its fun and instructive to re-visit the great works of those who have gone before us. We study these books to open and enrich our minds. Every year I attend the Shakespeare Festival in Cedar City, Utah. Before we attend any of the plays, there is an orientation seminar to help us get the most out of the play. The next day, there are hour-long seminars where we can exchange ideas about what we saw the night before. These seminars are extremely well attended. However, to discuss a book, one doesnt need a large group for effective exchange of ideas. In Tucson, there are many such groups that discuss books. We can do this on a one-on-one basis. We have a group here that informally discusses Lee Childs books because we love his character Jack Reacher. This is fun and often forces us to re-read parts of his books searching for things we may have missed in our initial reading. Fans of certain authors can get together to discuss the story or ideas. The lamp of learning always burns brighter when we listen as others add new and different perspectives. Do you have any ideas on a book discussion you would like to host? Remember that even visually impaired readers can join in discussions because of audio books.
