1.0 Health and Safety Policy During construction activities JV Pihl/Lackeby-Purac is working to achieve the following health and safety objectives: Zero injuries to personnel Zero material or property loss An efficient emergency response organization

To achieve the objectives in our health and safety policy the following areas will be prioritized in all our construction activities. The management will demonstrate its corporate responsibility and commitment to promote occupational health and safety by preserving and developing human and physical resources. The management ensures the deployment of financial resources required to implement and promote the health and safety policy on each new construction site. The management will actively demonstrate and promote the health and safety policy through its daily work. Health and safety shall be considered in all decisions taken. All construction activities shall as a minimum meet the applicable national laws and local regulations. All employees shall receive the required training enabling them to do their work safely. Subcontractors and suppliers shall be evaluated on their health and safety performance and shall comply with our health and safety policy. It is the responsibility of all employees to work within the objectives of our health and safety policy.

2.0 General JV Pihl / Lckeby-Puracs H&S system has been developed in accordance with OHSAS 18001. The principles are shown in figure 1.

Fig. No. 1: JV Pihl / Lckeby-Puracs H&S management system The H&S Plan and its regulations apply to any employee, employed by JV Pihl / Lckeby-Purac, sub-contractors, suppliers, the Client, guests and any other third person requiring access to JV Pihl / Lckeby-Puracs construction site. Further reference is made to the Factories Act, Factories regulation 1961 and the Health, Safety and Welfare Regulation of Sri Lanka.

3.0 Scope of works The project comprises the construction of two wastewater disposal systems in JaEla/Ekala and Ratmalana/Moratuwa. The Contractor shall design and build the Wastewater Disposal Systems for the two project areas. The design shall facilitate future expansions and connections. The Tender Documents give a design of the major pipelines, but the Contractor shall check the design and make his own updating and additional design in accordance with

the Employers requirements. The Contractor will design the Collection System and build it in accordance with the design.

4.0 Organisation 4.1 Organisation Plan for H&S The organisation plan for the construction site is attached in appendix 2. 4.2 Responsibilities The Project Director has the overall responsibility for implementation of JV Pihl/lackeby-Purac Health & Safety policy within the project. The Project Director has the responsibility to ensure that the Project Manager is following up implementation of the Health & Safety policy, and in co-operation with the Project Manager, make the needed resources available. The Health & Safety Manager will assist in implementation of the Health and Safety Policy within the project and monitor the efficiency. The Project Manager is responsible for the flow of information, affecting the transfer of the knowledge & requirements needed in respect of working environment and safety, to the supervisors, site engineers, sub-contractors and suppliers.

The delegated Health & Safety Manager shall report to the Project Manager and be responsible for preparation of Health & Safety Plan, implementation, monitoring and follow up on the implementation in the site organisation. The Health & Safety Manager will assist the Site Engineers in implementation of the H & S policy on the site. The Site Engineers / supervisors report to the Project Manager and are responsible for ensuring that the employees follow the H & S procedures. Election of safety representative is executed according to the local requirements. As a minimum the safety group on site will be the Site Engineer and the Health & Safety Officer. The Project Manager can nominate the Production Manager in his absence. The Safety Group has the responsibility to execute a weekly Health & Safety Inspection and to decide on any Health & Safety issues related to the construction activities.

5.0 Administrative requirements 5.1 H&S Coordination and responsibilities JV Pihl/Lackeby-Purac has by signing the contract taken over the responsibility for health and safety within the conditions of the contract. The responsibility comprises own and/or sub-contractors work. The Health and Safety Plan describes how JV Pihl/Lackeby-Purac will manage and coordinate the Health and safety work within the scope of works. The conditions of the contract will be incorporated in the H&S Plan as well as any national laws and regulations. JV Pihl/Lackeby-Purac will participate in the Clients Health & Safety meetings and inspections as specified in the conditions of contract. JV Pihl/Lackeby-Puracs safety group will carry out weekly H&S Inspections of subcontractors work on the construction site. During the inspection any findings will be discussed and corrective action agreed. The results will be documented by the H&S inspector in an inspection form appendix no 3 and will be available to the Client upon request. The inspection form will be distributed to the supervisors and any subcontractor for action. Observed hazards will be informed immediately in an Observed Hazard Form appendix 3.2. and rectify. 5.2 Risk assessment Procedures for Managing Identified Risks Risk assessment will be carried out in three steps: 1) Risk assessment will be carried out during the planning process. Any high-risk activities shall be identified when starting a project. For the activities evaluated as high-risk activities, the management will decide on preventive action to be taken and agree on safe working method in order to minimize the risk.

2) Work procedures shall be executed for activities identified as high-risk activities. The work procedure shall describe the execution of the works, responsibilities, risk assessment and preventive actions to be taken. 3) A safe job analysis shall be performed in order to ensure that the employees are correctly instructed before starting high-risk activities. The safe job analysis shall be performed by the site engineer together with subcontractor supervisor, foreman assisted by the H&S officer and with the subcontractors employees being directly involved in the operation. The Safe Job Analysis will serve as an instruction on the job for the employees. Any risk shall be identified during the instruction and information on preventive action as well as personal protective equipment to be used shall be provided. Procedure and Risk assessment forms can be found in appendix 6. 5.3 Working hours Typical working hours are sixty (60) hours per week, or as required, to comply with both the contractual obligations and the local labour laws, regulations and agreements. 5.4 Investigation of incidents and accidents Incidents and accidents are investigated and the information given in the Form attached in appendix 4. The report is forwarded to JV Pihl/Lackeby-Puracs Head office and The Employer. The results of the investigations will be discussed in the H&S Group and corrective action agreed. The production manager is, as head of the H&S group, in charge of the accident investigation. Any severe accident will be investigated in co-operation with the relevant authorities. 5.5 Health and Safety requirements for sub-contractors and suppliers Any sub-contractor or supplier has by signing agreement with JV Pihl/Lackeby-Purac agreed to implement the Health- and Safety requirements set out by the Client and JV Pihl/Lackeby-Purac for the Project. All Health and Safety Plans, procedures and instructions issued by the Client and JV Pihl/Lackeby-Purac shall be followed. 6.0 General Site-related conditions 6.1 Announcement for start of work Prior to start of the construction works JV Pihl/Lackeby-Purac will forward a written announcement to the Client informing him of the start date. The Client will forward an announcement to the local authorities in accordance with the local legislation. When the Client has transferred the coordination responsibility to JV Pihl/LackebyPurac, the information shall be forwarded directly to the local authorities. 6.2 Security, Guard system and access control

A detailed Site specific Security plan will be prepared before mobilization. Fencing and vehicle/ personnel gates will be installed in agreement with the Employer. The security control will include access control of all personnel including JV Pihl/Lackeby-Purac employees, Subcontractors, visitors, and Company representatives who enters the construction site. The main function of the Security will be: Restrict entry of unauthorized personnel and vehicles onto the project or work site Check ID- Pihl/Lackeby-Purac employees, Subcontractors will carry company ID cards. Inspection of material entering and leaving the site Maintaining a personnel and Vehicle Log Patrolling the site during working hours and after normal working hours Direct vehicles or equipment to a pre-designated on-site location Direct emergency vehicles Maintain daily security report files. Enforce parking area regulations and site speed limit 6.3 Personal Protective Equipment (PPE) In addition to any local requirements the following PPE shall be worn on any of JV Pihl/Lackeby-Purac s construction sites: Safety helmet Safety shoes with steel toe and nail protective sole

All PPE shall be CE approved and comply with EN 471 or similar. The H&S officer will specify the PPE requirements for each category of work. All PPE shall regularly be maintained and kept in good condition. Worn-out or damaged PPE shall be returned for repair or disposal. Sub-contractors shall provide necessary and well-maintained PPE for their own employees. The subcontractors H&S officer or the employee responsible for the store shall control the PPE handed out and keep an updated file with information on the PPE handed out to each employee.

Safety helmet Safety helmet is mandatory when entering any of JV Pihl/Lackeby-Puracs construction sites. Only exception will be the access from the gate to the site office and labour sheds when passage is on marked gangway without any nearby works in the height. Machine operators shall have their safety helmet available in their machine for use when exiting the cabin.

Safety shoes Safety shoes with heel counter, steel toe and nail protective sole is mandatory when entering any of JV Pihl/Lackeby-Purac s construction sites. Only exception will be the access from the gate to the site office and labour sheds when passage takes place on marked gangway without any nearby works. Face shields Face shields are mandatory for work with chemicals, injection works, concrete, highpressure tools and electric tools, cutting, grinding and chipping. Welding goggles Welding goggles are mandatory during welding and use of blowtorch. Hearing protection Hearing protection is mandatory when working in areas with a noise level exceeding 85 dB(A). Gloves Special gloves are mandatory when working with chemicals, oil, during welding and use of blowtorch. Harness Harness is mandatory for work over water and when working from a height exceeding 1,8 m and when the work cannot be executed from scaffolding or lift. Respiratory protection Respiratory protection shall be used when working with dust creating activities or chemicals. The type of filter or fresh-air supply to be used shall be according to the risk assessment/safe job analysis performed prior to the start of work. When in doubt whether respiratory protection shall be used or not, the H&S inspector shall perform an evaluation of the activity. Working clothes Working clothes will be handed out in accordance with the established procedure for the actual project. Mechanics, welders and employees working with hot work shall wear non-flammable clothing, i.e. cotton-based fabric. Life vest Life vest shall be used when working from boats or above the sea when there is a risk of falling into the water. Visibility vest Visibility vest shall be worn if work carried out in public areas, for road jobs and for special job functions with need of visibility e.i riggers. 6.4 Emergency equipment In each Treatment Plant area the following emergency equipment shall be available:

Fire extinguisher, 6 kg Fire blanket Eye rinse First-aid kit Stretcher

6.5 Emergency Plan An Emergency Plan covering the construction site will be prepared. A mandatory H&S introduction course will be held where all employees will be informed of the content in the Emergency Plan. Drills will be executed in accordance with the contract requirements to test the Emergency Plan. The drills will be coordinated with the local authorities and the Client. However, the first drill shall always be internal. When cranes are used during drills for moving stretchers a dummy will be used in the stretcher with the equivalent weight of a person. 6.6 Signs The entrance to the site shall be marked with JV Pihl / Lckeby-Purac logo. Signs shall be placed for first-aid equipment, fire fighting equipment, pressure bottles, helmet area, safety shoes area and warnings signs where needed. The signs shall fulfil the requirements stated in the contract. Standard CE approved signs can be printed on colour printer and laminated. Text will be in English, Sinhala and Tamil.If signs are of such character, readings of the text are necessary for the essential understanding. 6.7 Fire prevention Fire extinguishers shall be placed in the Workshops, Stores and Site Offices according to the requirements of the building legislation and after agreement with the Fire Department. The Safety Manager will inspect the fire fighting equipment and an external inspection will be performed every 12th month. The workshop area and store area shall be kept tidy in order to prevent fire. 6.8 Materials and Chemical storage Chemicals shall be stored secured from spillage to the ground, drainage and sewerage system. The storage shall be arranged so that the content of the largest drum stored can be contained within the area with no risk of seepage to the ground. Oil absorbent material will be available in the workshop for cleaning of any spill.

The suppliers Material Safety Data sheet shall be kept in an updated file where all chemicals used on the project are filed. The file shall be available within the storage area.

6.9 Alcohol, drugs, firearms With reference to JV Pihl/Lackeby-Puracs Health and Safety Policy the following regulations are valid for all employees, sub-contractors and third person on JV Pihl/Lackeby-Purac s construction sites: Possession, use or sale of alcohol and drugs on the construction site, as well as entry to the site while under the influence of alcohol or drugs, are prohibited and are dismissal conditions. Any employee regularly using medicine shall document the need from a doctor. The foreman and the employees closest colleagues shall be informed of the situation. Possession, use of firearms or any other types of weapons including explosives and ammunition on the construction site is prohibited and is dismissal conditions.

6.10 Housekeeping and disposal of Waste Materials Each employee is responsible for maintaining his work site clean. Waste materials shall be removed daily and placed in the waste containers. The waste materials shall be sorted according to the local laws and regulations. Chemical waste shall be placed in an area allocated for this. If sub-contractors fail to fulfil the above requirements JV Pihl/Lackeby-Purac will execute the cleaning and the cost will be invoiced the responsible sub-contractor. 6.11 Excavation Activities Sloping, benching, shoring, shielding, or other protective systems are required to protect personnel from cave-ins except when the excavation is made entirely in stable rock or is less than 1.5 meters deep and there is no indication of possible cave-in, as determined by the excavation competent person. Cave-in protection is required whenever a worker must have head and chest below grade (for example, cementing pipe joints on hands and knees in a 48-inch deep utility trench). Trenches greater than 1.2 meters deep shall be provided with a ladder, stairway, or ramp positioned so that the maximum lateral travel distance is no more than 10 meters. Protective systems for excavations deeper than 6 meters must be designed or approved by a registered professional engineer. Walkways shall be provided where personnel are required or permitted to cross over excavations. Walkways above lower levels shall be equipped with standard guardrails.

. No one will be allowed to enter the excavations unless completely necessary, and only after the competent person has completed the daily inspection and has authorized entry. An inspection shall be conducted by the competent person prior to the start of work, as needed throughout the shift, after every rainstorm, and after any hazard increasing occurrence documentation must be maintained onsite at all times. Follow all excavation entry requirements established by the competent person. No one will be allowed to enter excavations where protective systems are damaged or unstable. No one will be allowed to No one will be allowed to enter excavations where objects or structures above the work location may become unstable and fall into the excavation. No one will be allowed to enter excavations with the potential for a hazardous atmosphere until the air has been tested and found to be at safe levels. No one will be allowed to enter excavations with accumulated water unless adequate precautions have been taken to protect employees against the hazards posed by water accumulation.

6.12 Fall protection, Guardrails and openings For works being carried out above water or the height 1,8 m. or more above ground level or if a fall onto the surrounding base would involve danger, protection against fall must always be established. Protection must normally consist of guardrail, scaffolding or personnel lifts. If such precautions cannot be taken, fall protection equipment must be used. The type of fall protection equipment must suit the job to be done, fulfil its purpose and be CE marked. Harness with D ring must mainly be used. The equipment must be carefully checked before it is taken into use (even if it is new). Only persons trained by a skilled person must use fall protection equipment. All holes in deck must be covered or screened off and be protected from displacement. 6.13 Scaffolds and ladders Scaffolding higher than 3 metres must only be erected and taken down by personnel trained in this type of work. When mounting of the scaffolding has been finished, an official approval tag must be placed at all access points. Ladders must be secured at the top and secured at the bottom and they must be erected at an appropriate inclination and in such a way that it is possible to reach with your hand at approx. 1 m. above the upper level. The height of a ladder must not exceed 5 metres.

6.14 Confined space entry All work in a confined space requires special safety planning. A Safe job analysis must be conducted, atmospheric testing and ventilation must be established, and a buddy and emergency system must be planned. A permit must be issued before entrance and work in a confined space. 6.15 Lifting Cranes brought to the site are only to be erected by specially trained personnel and must be load tested whenever they are being relocated (not mobile cranes). Only operators with a valid certificate for the particular crane must operate the crane. Heavy lifts, the use of special rigging equipment and difficult lifts such as twin lifts requires a safe job analysis Rigging must only be done by personnel who have gone through rigging training. Only lifting equipment that is marked with SWL or WLL may be used. 6.16 Machines All machines, technical equipment on the site must be approved, minimum CE marked and properly maintained. Instructions for the machine must be available and must only be used by personnel who have been trained and are confident in the use of them. Lockout and tag out procedures must be put in place for any repair or cleaning activities of the machines. 6.17 Heavy construction machines Heavy construction equipment must only be operated by operators with valid certificates. An internal verification should also be done to verify the operators skills Reverse alarms must be fitted to all construction equipment. 6. 18 Traffic and transport The system for traffic regulation, including signalling, traffic signs and organisational responsibilities, will conform with all the requirements of the Roads and Police Authorities In accordance with government regulations. All local and site traffic regulations must be obeyed when driving on public roads or inside the perimeter of the site. The driver must be in position of a valid driving license. The vehicle must only be operated if it is road worthy.

Reverse alarms must be fitted to trucks and other construction equipment. Passengers must be limited to seating provided and seat belts must be worn where provided. A spotter/flagman must be provided with high visibility clothing to guide vehicles for example when backing up in congested areas or when vehicles are obstructing other traffic. 6.19 Theft and vandalism In order to prevent theft all equipment shall be clearly marked. The subcontractor is responsible for the site office, workshop and storage containers being locked outside working hours. Machine supervisors are responsible that the keys are being removed from machines when not used and that the machine is being locked. The workshop supervisor is responsible for the diesel pump/storage being locked outside working hours. In order to prevent theft of working clothes and personal protective equipment records shall be made and kept updated for such equipment handed out to the employees.

7.0 Communication 7.1 Routines to make requirements known to own employees Sub-contractors are responsible for informing their own personnel and for sending them to the mandatory safety introduction courses. Information on health and safety regulations valid for the Project is given to employees through: H&S Introduction and training course Toolbox meetings Safety meetings Safety inspections

Sub-contractors are responsible for informing their own personnel and for sending them to the mandatory safety introduction courses. 7.2 H&S Introduction Course All personnel employed for JV Pihl/Lackeby-Puracs construction sites shall participate in a Basic Health and Safety Introduction Course. The objective for the Introduction course is to make the employee familiar with the construction site and the rules and regulations to be followed.

The Introduction course shall be arranged by the health and safety Manager. Please refer to section 8 for description of the course. 7.3 Personnel Site Safety handbook All supervisors, foremen will in connection with the HSE introduction course receive a personnel Site safety handbook with containing basic to the most basic health, safety and environmental requirements. 7.4 Toolbox meetings The H&S officer shall in cooperation with the supervisor hold a weekly toolbox meeting for all workers. Subjects to be informed at the toolbox meeting are incidents and accidents during the past week and any new H&S information received from the Client. In order to have an efficient toolbox meeting the H&S officer shall prepare a short agenda. The meeting is not to exceed 15 minutes and only subjects related to health and safety can be discussed at the meeting. The toolbox meeting is to be held in each individual supervisors work area and meeting time should be in connection with lunch break or start of work. 7.5 Safety meetings The H&S manager shall in corporation with the Site Engineer or the Supervisor hold weekly safety meeting for each area. Agenda for the meeting shall be: Comments to last m.o.m Sub-contractors manning Accidents and incidents Revisions to the Health and Safety Plan Information re. inspections from the authorities (Labour Inspection, Police, Fire Service, Client Safety in general Future Work operations where special safety measures shall be taken Any other issues Next meeting

7.6 Health and Safety Inspections Documented Health and Safety Inspections shall be carried out on a weekly basis covering all activities on the construction site. The Inspections are carried out using the Inspection Form in Appendix 3. Participants in the inspection shall be the area responsible site engineer or Supervisor, subcontractors safety representative and the H&S manager. It is important that the participants during the inspection can take immediate action on deviations from the HSE regulations and thereby correct the deviations before they develop to accidents. The results of the inspections shall be documented by description of observations and the corrective actions to be taken including dead lines If possible, photo documentation shall be used. The Inspection Form shall be distributed to the participants the day the inspection has been performed.

Daily health and safety inspections are carried out by the H&S officer.

Inspection and Certification of Equipment and Vehicles Inspections for the individual equipment and vehicles will be executed according to manufacturers recommendations or specifications. Required certification for vehicles and equipment will be obtained following necessary legal procedures to obtain such certificates. A responsible person with the necessary technical background will be appointed. The responsible person will ensure that all equipment is mechanically sound, well maintained, ensure that certificates of the equipment and vehicles are complete and up to date. If unsafe or defective vehicle/equipment is found, it will be immediately halted from operations, tagged with defective sign and reported to supervision or Construction manager. Prior to sending the vehicle or equipment to operations, necessary corrective actions will be taken. Inspection of lifting equipment and electrical installations shall be included in the weekly H&S inspections of the site. Site Visits by Senior Management It is within company practices that senior management visit work sites. During site visits, senior management will observe site H&S conditions to ensure that works are executed under company H&S policies and contract requirements. If non-compliance conditions against company H&S policies are found, the management will inform the Project Manager or the H&S group to take immediate corrective actions as necessary. Reporting of the irregularities will be done trough the weekly inspection forms and will be reported to the project manager and the head office through monthly H&S report. 7.7 Communication with third person All communication with third person shall be via the Client. JV Pihl/Lackeby-Purac s and subcontractors employees cannot make any statements to the media without prior approval by the client. 8.0 Health and Safety Training Programme 8.1 Basic health and safety introduction course A basic Health and Safety introduction course will be held for all new employees Including sub-contractors personnel. The employees shall complete the course prior to start of work. A register for the employees having participated in the course shall be kept in the Personnel file. The management is responsible for collecting copies of certificates for their employees as well as for any of their sub-contractors employees.

Reference to appendix no. 9 for introduction course agenda. 8.2 Other Health & Safety Training Courses and training shall be held in order to comply with the local legislation and the contract conditions. As a minimum there shall be an employee trained in first aid in each working area. The supervisor is responsible for collecting copies of required certificates for their employees as well as for any of their sub-contractors employees. The documentation shall be kept with the personnel register. 9.0 Reporting and documentation 9.1 General All reporting to any authority or 3rd party shall be done after prior approval by the Company representative. 9.2 Reporting to the authorities Reporting of accidents and incidents to the authorities shall be done using their forms. Copy of the form is to be sent to the Project manager at JV Pihl/Lackeby-Purac Head Office for further elaboration. A copy is forwarded to the Client upon request 9.3 Reporting to the Company Reporting of all accidents /Incidents to the Company representative shall be done within 24 hours. Accidents shall be informed to the Company representative on the same day. Any reporting to the Company is subject to the Project Managers approval. Copy of the reporting is to be forwarded to the Project Director. Any severe accident with injury to personnel shall be reported to the Project Manager immediately. On a monthly basis the following Health and Safety information is to be forwarded to the Project Manager, the H&S Manager and Employer, Appendix No 8. Number of Lost time accidents and corresponding number of lost days Number of Accidents with no lost days Number of incidents (First aid accidents, near-misses, Material damage or environment damage.) Accident trend of the month Preventive actions planned next month

The H&S Manager is responsible for compiling all H&S reporting to the Client, Project Manager and the authorities. An updated file with copies of all reporting will be kept at the site. Definitions: AN ACCIDENT is an unplanned, uncontrolled event, which has resulted in personnel being harmed in some way. Mainly caused by human error and/or high-risk conditions. AN INCIDENT is an unplanned, uncontrolled event, which has resulted in damage to material etc, rather than injuring a person. A NEAR MISS (is recorded as a Incident) is an unplanned, uncontrolled event, which, under different circumstances, could have been an accident (i.e. hurt someone), or incident (i.e. damaged property) but did not in this case.

Revision and distribution of the Health and Safety Plan Revision of the Health & Safety Plan Main Revision When the entire Health & Safety Plan (H&S Plan) has been revised, all pages shall be supplied with the new revision number. The total number of pages of the H&S Plan is then updated. The actual revision number will be added to all pages including the front page. The site specific H&S plan is distributed to: Head office Project manager Site managers H&S manager Supervisors Client

Instruction for holders of the H&S Plan When revisions are distributed it is the responsibility of each receiver to familiarize himself with the changes. A list of revisions will be included (Appendix 1) corresponding with the following: date, description of revision, pages revised, revision number, revision executed by, revision verified by, revision approved by.
